Patient adherence to voice therapy presents a problem in voice treatment. Although voice therapy has demonstrated efficacy for a variety of disorders, it is limited by poor patient adherence to practice and generalization of a target voice technique between sessions and high drop-out dates. Typically, patients can produce this target technique with therapist assistance during their treatment sessions. However, they experience difficulty in replicating and sustaining it independently between sessions and after discharge from therapy. Thus, from the perspective of self-regulation and social-cognitive theory, adherence failure is related to lack of feedback and self-regulatory support between sessions. Popular, affordable iOS-based devices have the capability to provide such feedback and support anytime, anywhere. Likewise, such devices can be used to covertly or overtly monitor patient practice and generalization of a target voice technique. In 2 separate studies and one pilot investigation, results indicated increased patient practice, generalization, or accuracy of voice production in absence of clinician assistance when mobile assistance was compared to a standard of care condition. These results suggest that mobile support may play a substantial role in resolving adherence and maintenance challenges, and have the potential to move voice care from a periodic to a continuous model.
