Assessment of Postmenopausal Bleeding

چکیده
Menopause, the end of ovulation and menstrual periods, naturally occurs for most women at the age of 40–55 years. The process of ending ovulation and menstruation is gradual, spanning 1 to 2 years. Postmenopausal bleeding (PMB) is bleeding that occurs 12 or more months after the last menstrual period and accounts for 5% of all gynecologic office visits1. Moreover, a reported 25% of gynecologic surgeries involve abnormal uterine bleeding2. The response for the postmenopausal individual will depend on her own specific endometrial sensitivity, as well as the magnitude of the dose of the hormone supplement. Approximately one in 10 women will experience some bleeding after the menopause3. Postmenopausal bleeding should always be taken seriously and be investigated, no matter how minimal or nonpersistent. Causes may be nongenital, genital extrauterine or uterine4. Possible uterine conditions associated with PMB include endometrial atrophy, polyps, estrogen therapy, foreign bodies, trauma, infection, endometrial hyperplasia, and carcinoma (Table)4,5. Uterine myoma should never be accepted as a cause of PMB. Endometrial atrophy is the most common endometrial finding in women with PMB, accounting for 60–80% of such bleeding. Women with endometrial atrophy have usually been menopausal for 10 years. Hormonal replacement therapy accounts for 15–25% of PMB. The uterine lining responds to supplemental hormone stimulation just as it did earlier in the woman’s ASSESSMENT OF POSTMENOPAUSAL BLEEDING

ACOG Committee Opinion No. 440: The Role of Transvaginal Ultrasonography in the Evaluation of Postmenopausal Bleeding.
The clinical approach to postmenopausal bleeding requires prompt and efficient evaluation to exclude or diagnose carcinoma. Women with postmenopausal bleeding may be assessed initially with either endometrial biopsy or transvaginal ultrasonography;this initial evaluation does not require performance of both tests.Transvaginal ultrasonography can be useful in the triage of patients in whom endom...
متن کاملInvestigation of women with postmenopausal uterine bleeding: clinical practice recommendations.
Postmenopausal uterine bleeding is defined as uterine bleeding after permanent cessation of menstruation resulting from loss of ovarian follicular activity. Bleeding can be spontaneous or related to ovarian hormone replacement therapy or to use of selective estrogen receptor modulators (eg, tamoxifen adjuvant therapy for breast carcinoma). Because anovulatory "cycles" with episodes of multimont...
متن کاملUterine inversion in postmenopausal age: case report
Background: Nonpuerperal uterine inversion is a rare medical condition that many gynecologists might not be encountered even with one case during their entire medical practice. It refers to the expulsion of uterine corpus from the dilated cervix resulting in uterus being turned inside out. There are two kinds of uterine inversion, puerperal and non-puerperal, which the second condition is less ...
متن کاملThe use of new and old ultrasound techniques in the assessment of women with postmenopausal bleeding
Introduction Postmenopausal bleeding (PMB) is the most common symptom of the endometrium. Cervical cancer must also not be forgotten, especially in women with a thin endometrium. Therefore, all women presenting with PMB should undergo evaluation with transvaginal ultrasound to estimate the risk of endometrial cancer, and speculum examination/cervical cytology to rule out cervical pathology. Man...
