چکیده

IT IS now over 30 years since Sheldon's classical monograph on haemochromatosis, but in spite of much intensive research, many aspects of this condition remain enigmatic. We have recently observed a case of haemochromatosis secondary to sideroblastic anaemia in which the appearance of the clinical signs of haemochromatosis were unusually rapid and paralleled by a dramatic and fatal deterioration in the patient. In the later stages of the disease the patient became excessively pigmented and was observed to pass jet-black urine. Melanogens were identified by the laboratory. In a comprehensive review of the literature we found only six reports of melanogenuria occurring in the absence of melanotic

Sideroblastic and megaloblastic change.

Introduction THE simultaneous occurrence in the bone marrow of megaloblasts and ring-sideroblasts may result from a variety of causes. In a series of seventy patients with sideroblastic anaemia, MacGibbon and Mollin (1965) observed megaloblastic change in 544O of cases of primary sideroblastic anaemia, and in 74YO of cases of secondary sideroblastic anaemia. The megaloblastic change was attribu...

A case of paroxysmal nocturnal haemoglobinuria associated with secondary haemochromatosis, a lower nephron nephrosis, and a megaloblastic anaemia.

Secondary or exogenous haemochromatosis has been defined by Aufderheide, Horns, and Goldish (1953) as a condition acquired as " a consequence of anaemia, blood transfusion, or both, and characterized by increased hepatic and total body iron and unequivocal portal cirrhosis." These authors found only 20 cases in the literature and has-e added two more of their own. They comment on the lack of ad...

Sideroblastic anaemia with iron overload presenting as an arthropathy.

We report on 2 patients with sideroblastic anaemia an secondary iron overload whose presenting symptom was an arthropathy of small an large joints. One patient had marked spinal symptoms which were severe enough to suggest ankylosing spondylitis, and the pattern of arthritis in the other resembled seronegative rheumatoid arthritis. We believe these to be the first cases described of an arthropa...

Iron overload despite partial gastrectomy.

Iron overload was found in 3 patients who had undergone partial gastrectomy: a 61-year-old woman developed iron overload because she may have had idiopathic haemochromatosis and had also been given parenteral iron; in a 62-year-old man with thalassaemia minor, iron overload may have developed because of increased oral iron ingestion, low serum folate, increased, albeit ineffective, erythropoies...

Idiopathic acquired sideroblastic anaemia transforming to acute myelosclerosis.

A case of idiopathic acquired sideroblastic anaemia transforming to acute myelofibrosis is reported. The appearance of atypical megakaryocytic proliferation in idiopathic acquired sideroblastic anaemia may presage the development of an acute myelofibrotic phase of this usually chronic disease.
