Weaning of the Patient with ARDS
نویسنده
چکیده
Assessment of weaning readiness The ability to separate successfully from mechanical ventilation is dependent upon a host of physiologic factors (Table 1). Central ventilatory drive must be adequate; the patient must be able to maintain a patent airway, if extubation or decannulation is contemplated; and the patient must be able to cough and adequately clear secretions. Airway resistance must be adequately low; airway edema or obstruction can preclude extubation. The work of breathing must be adequately low; decreased lung compliance, increased airway resistance, and hyperinflation impose an increased load upon respiratory muscles. The respiratory muscles themselves must possess sufficient strength and endurance. Even with normal respiratory muscle function, however, abnormal chest wall mechanics, such as severe hyperinflation, abdominal distention, or kyphoscoliosis can place respiratory muscles at a mechanical disadvantage, resulting in fatigue and respiratory failure.

Ecmo in a diabetic patient with cerebellar infarction and ARDS- A case report
Background: Extra corporeal membrane oxygenation (ECMO) has been considered as a crucial modality for the management of acute respiratory distress syndrome (ARDS). Since ECMO is considered to be an invasive modality and may be associated with some complications, there are strict indications for its use. Here we report successful treatment of a case of severe ARDS with Veno-Venous ECMO. Case pre...
متن کاملAcute Respiratory Distress Syndrome in a Patient With Suspected Influenza: A Case Report
Acute Respiratory Distress Syndrome (ARDS) is a clinical syndrome consisting of tachypnea, refractory hypoxemia, and diffuse opacities on chest radiographs after infection or trauma which ultimately leads to respiratory failure. The principles of treatment are based on patient care in ICU, mechanical ventilation and medical treatments. By using lower plateau, less tidal volume, higher positive ...

Several papers on respiratory and critical care published from March 2012 to February 2013 were reviewed. From these, this study selected and summarized ten articles, in which the findings were notable, new, and interesting: effects of high-frequency oscillation ventilation on acute respiratory distress syndrome (ARDS); safety and efficacy of hydroxyethyl starch as a resuscitation fluid; long-t...
متن کاملMechanical ventilation in ARDS: a state-of-the-art review.
Mechanical ventilation is an essential component of the care of patients with ARDS, and a large number of randomized controlled clinical trials have now been conducted evaluating the efficacy and safety of various methods of mechanical ventilation for the treatment of ARDS. Low tidal volume ventilation (</= 6 mL/kg predicted body weight) should be utilized in all patients with ARDS as it is the...
متن کاملWork of breathing as a weaning parameter in mechanically ventilated patients.
STUDY OBJECTIVE Predicting patient responses to changes in ventilatory support or weaning is primarily based on bedside measure of respiratory mechanics and clinical judgement. Work of breathing (WOB) has been considered to be the best predictive factor in weaning. This study evaluated patient WOB as a predictive measure of weaning and extubation. DESIGN Blinded, descriptive evaluation of the...
متن کاملمقایسه نسبت Spo2/Fio2 با نسبت Pao2/Fio2 در بیماران مبتلا به آسیب حاد ریه و سندرم دیسترس حاد تنفسی
Background & Aims: Diagnostic criteria for acute lung injury (ALI) and acute respiratory distress syndrome (ARDS) requires acute onset of disease, chest radiograph demonstrating bilateral pulmonary infiltrates, lack of significant left ventricular dysfunction and Pao2/Fio2 (PF) ratio 300 for ALI or 200 for ARDS. And the latter one requires invasive arterial sampling. Measurement spo2 by pulse...
