Cardiovascular Research Center, Tabriz University of Medical Sciences, Tabriz, Iran. (Received 21 May 2010; Accepted 17 Sep 2010) Background: The aim of the present study was to investigate the effects of the left ventricular outflow gradient on tissue doppler imaging (TDI) values in patients with hypertrophic cardiomyopathy (HCM). Methods: This prospective observational study was conducted in a tertiary referral heart hospital (Tabriz, Iran) from March 2004 to March 2008. Fifty two patients (28 men and 24 women) with definite diagnoses of HCM were selected for the study. 2D, M-mode echocardiography and TDI were performed for all patients. In TDI, the velocities of myocardium was measured at lateral and septal corner of the mitral valve annulus in peak early diastolic (Ea), peak late diastolic (Aa) and peak systolic (Sa). Then, the TDI velocities were compared in patients with left ventricle outflow gradient less and more than 30 mmHg. Results: The mean age of the patients was 42.13 ± 12.22 years. All patients were in New York Heart Association functional class I or II. The mean EF was 55 ± 7%. It was found that TDI velocities are reduced in patients with HCM, compared to normal vaues and Ea in septal corners of the mitral annulus was reduced more in patients with left ventricle outflow gradient > 30 mmHg (5.3±1.6 vs 7.1±1.2, P=0.01). Conclusion: TDI findings specially peak early diastolic (Ea) mitral annular velocity are good parameters in HCM patients assessment. Evaluation of left ventricular outflow tract obstruction with 4D phase contrast in patients with hypertrophic cardiomyopathy

Background Patients with hypertrophic cardiomyopathy (HCM) develop left ventricular outflow tract (LVOT) obstruction due to combination of asymmetric septal hypertrophy and systolic anterior motion of the mitral leaflet. Degree of LVOT obstruction is an important predictor in patient symptoms and clinical outcome. LVOT obstruction is currently assessed by Doppler echocardiography (echo) using p...

Intraoperative Doppler echocardiography in hypertrophic cardiomyopathy: correlations with the obstructive gradient.

Although significant pressure gradients can be recorded across the left ventricular outflow tract in patients with hypertrophic cardiomyopathy, controversy exists regarding the presence or absence of true obstruction. Ten patients with hypertrophic cardiomyopathy were studied at the time of septal myectomy. A sterile continuous wave Doppler transducer was placed on the ascending aorta and direc...

Mid systolic septal deceleration in hypertrophic cardiomyopathy: clinical value and insights into the pathophysiology of outflow tract obstruction by tissue Doppler echocardiography.

H ypertrophic cardiomyopathy (HCM) is an inherited cardiac disorder characterised by abnormal left ventricular (LV) hypertrophy in the absence of severe aortic stenosis and hypertension. The presence of a dynamic gradient in the left ventricular outflow tract (LVOT) above 30 mm Hg at rest is associated with an increased mortality. Surgical myectomy or transcoronary ablation of septal hypertroph...

Adverse effect of nifedipine on left ventricular obstruction detected by pulsed Doppler echocardiography.

The effect of nifedipine on the ejection flow pattern in the left ventricular outflow tract was observed using pulsed Doppler echocardiography in 6 patients with hypertrophic cardiomyopathy. After sublingual administration of nifedipine (10 mg) to 1 patient, the turbulent ejection flow pattern became more marked and increased in duration compared with the initial state. An increase in turbulenc...

Solitary accessory and papillary muscle hypertrophy manifested as dynamic mid-wall obstruction and symptomatic heart failure: diagnostic feasibility by multi-modality imaging

BACKGROUND Solitary papillary muscle (PM) hypertrophy is an unique type of hypertrophic cardiomyopathy (HCM), which is characterized by predominant papillary muscle hypertrophy sparing the rest of other left ventricular segments. It has recently drawn our attention about the mechanism of left ventricular mid-cavity obstruction and the influence of pressure gradient in the left ventricular outfl...
