چکیده

Introduction Neurally adjusted ventilatory assist (NAVA) is used to adapt mechanical ventilation to patient demand while unloading respiratory muscles. Titration methods are focused on sustained unloading of the diaphragm while maintaining a stable tidal volume [1]. It is hypothesized that the activity of accessory respiratory muscles can supply extra information about muscle unloading and patient comfort during NAVA ventilation.

Impact of prolonged assisted ventilation on diaphragmatic efficiency: NAVA versus PSV

BACKGROUND Prolonged controlled mechanical ventilation depresses diaphragmatic efficiency. Assisted modes of ventilation should improve it. We assessed the impact of pressure support ventilation versus neurally adjusted ventilator assist on diaphragmatic efficiency. METHOD Patients previously ventilated with controlled mechanical ventilation for 72 hours or more were randomized to be ventilat...

Respiratory muscle activity and patient–ventilator asynchrony during different settings of noninvasive ventilation in stable hypercapnic COPD: does high inspiratory pressure lead to respiratory muscle unloading?

INTRODUCTION High-intensity noninvasive ventilation (NIV) has been shown to improve outcomes in stable chronic obstructive pulmonary disease patients. However, there is insufficient knowledge about whether with this more controlled ventilatory mode optimal respiratory muscle unloading is provided without an increase in patient-ventilator asynchrony (PVA). PATIENTS AND METHODS Ten chronic obst...

Titration and implementation of neurally adjusted ventilatory assist in critically ill patients.

BACKGROUND Neurally adjusted ventilatory assist (NAVA) delivers assist in proportion to the patient's respiratory drive as reflected by the diaphragm electrical activity (EAdi). We examined to what extent NAVA can unload inspiratory muscles, and whether unloading is sustainable when implementing a NAVA level identified as adequate (NAVAal) during a titration procedure. METHODS Fifteen adult, ...

Normalized Electromyography of Shoulder Muscles during Selected Functional Tasks of Upper Limb in Patients with Shoulder Impingement Syndrome

Background & Aims: Precise muscle activity pattern is required to maintain normal shoulder function and any alteration in muscle activity can result in movement impairment. The purpose of this study was to assess normalized electromyography (EMG) of shoulder muscles during selected functional tasks of upper limb in patients with shoulder impingement syndrome. Methods: Test group consisted of 15...

Diaphragmatic electrical activity: a new tool to assess lung hyperinflation?

447 September 2014 “ EXPIRATION and slow decrease of light together reaching minimum together ... in about 10 s and immediately cry as before. Silence and hold for about 5 s.” In the script for his late play “Breath,” Samuel Beckett conveys in sound and light his awareness of long and vital expiratory times. His personal experience with emphysema might have provided some of the inspiration. In ...
