G ALIO)1P rhythmi rel)esellts one of the minost important but still veryconfused aspeets of elinical auscultation of the heart. Most gallop rhythms are uinre(ogniized or misillterprete(l. This is unfortunate because a gallop, depending on the type, often represents the earliest elinieal evideiiee of (ongI.tive heart failure. After anialyzing the varloiis (lassifieatiolns of gallops1-21 we hve used( the followincg clinical elassificationi which We eoisider physiologic, praetical, ald simplle. (allops are first divided illtO systolic allld liastolic gallops (fig. 1). The systolic gallop (or click) is aii extra sound occurring in systole, generally lb-st hteard at the apex and usually represe(ntintg a b)eniigni findinig.7' . 15 Its exact etiologyN, is 0lot knUown, but in a number of cases appears to l)e extracardiac in origin. A few of the patients with this benign systolic sound have a )ast history of severe pneumonia, pleurisy, or l(,ss (c011111loiolv emlpyea. 011( could speculate ill suell cases as to the possibility of adhUsionls (.olltril)utillgf to this sound. The majority, however, have a completely negative hlistory. In addition to this benigni type of systolic sound there may be systolic soundds llear(l in conjunction with organic heart disease. For example, in aortic inisuifficien(cy a systolic sound may be heard at the apex, base, anid leripheral arteries which has beeni ternmed a systolic ejection sound, "'pistol shot,'' systolic click, or gallop.5 6. 1:1. 14 W\hen this is present (e.g., with aortic insufticiency) oiie usually has no difficulty iii deternuiiing the presence of orgralic heart (lisease. Triple Heart Rhythm.
Triple heart rhythm stands for the cadence produced when three sounds recur in successive cardiac cycles, just as two sounds compose the familiar dual rhythm of cardiac auscultation, and more rarely, four sounds a quadruple rhythm. The conflicting views on the subject have long served to discourage attempts at a clinical perception of the problem. Disagreement is perhaps best illustrated by rec...
متن کاملThe clinical significance of gallop rhythm in hypertension.
Gallop rhythm is widely held to be a sign of prognostic value, and this opinion has been supported by follow-up studies of patients with gallop rhythm in groups of mixed etiology. This method of assessment is, however, open to criticism; first, because it is so dependent upon the way in which the patients are selected, and secondly, because the prognostic significance of gallop rhythm is clearl...
متن کاملExperimental gallop rhythm in sheep with Gousiekte: correlation of changes in amplitude with haemodynamic parameters.
To investigate the correlation of haemodynamic parameters with the intensity of the gallop sound (S3), use was made of right heart catheterization with a Swan-Ganz catheter to measure the pulmonary and right atrial pressures. The cardiac output was determined with the thermodilution method. A radiocardiogram was obtained after a bolus injection of technetium pertechnetate. The cardiopulmonary f...
متن کاملMotions of the running horse and cheetah revisited: fundamental mechanics of the transverse and rotary gallop.
Mammals use two distinct gallops referred to as the transverse (where landing and take-off are contralateral) and rotary (where landing and take-off are ipsilateral). These two gallops are used by a variety of mammals, but the transverse gallop is epitomized by the horse and the rotary gallop by the cheetah. In this paper, we argue that the fundamental difference between these gaits is determin...
متن کاملThe Role of Heart Sounds Recording and Analysis in the Dyspneic Ed Patient
Extra diastolic heart sounds are produced as a result of increased stiffness and decreased compliance of the left ventricle. The third heart sound (S3) occurs 0.12 to 0.16 seconds after the second heart sound in early diastole (Figure 1). Of the many proposed theories, the most likely explanation is that excessive rapid fi lling of a stiff ventricle is suddenly halted, causing vibrations that a...
