Teaching NeuroImages: Multicompartmental intracranial hemorrhage in a pediatric patient.

چکیده
A previously healthy 9-year-old girl presented with sudden-onset headache followed by confusion. In the emergency department, she was hypertensive, bradycardic, and no longer responsive to any stimuli. An emergent head CT scan demonstrated a left frontal intraparenchymal hematoma with intraventricular and subarachnoid extension as well as subfalcine herniation (figure). She underwent decompressive hemicraniectomy, and digital subtraction angiography confirmed a 7-mm left middle cerebral artery aneurysm (figure). Intracranial hemorrhage in a pediatric patient that traverses multiple compartments (intraparenchymal, subarachnoid, and intraventricular) has a short differential that should include trauma, coagulopathy, vascular abnormalities (middle cerebral artery aneurysm, arteriovenous malformation, dural arteriovenous fistulas, and reversible cerebral vasoconstriction), venous thrombosis, and cocaine abuse.

Teaching neuroimages: large vagal nerve schwannoma presenting with hemorrhage and respiratory failure.
A 30-year-old man was intubated after sudden severe headache, loss of consciousness, and apnea. CT showed acute hemorrhage in the posterior fossa, brainstem compression, and intraventricular extension with early obstructive hydrocephalus (figure 1A). MRI showed a large heterogeneously enhancing extra-axial tumor extending from the cerebellopontine angle to the foramen magnum (figure 1, B and C)...

A 45-year-old man had a severe traumatic brain injury (TBI) withmulticompartmental hemorrhages (figure 1). He was initially noted to be awake and following commands with his right side. Two weeks later, his examination deteriorated to coma with flaccid quadriplegia. Initial workup, including EEG, was unrevealing. MRI brain showed new multiterritorial infarcts (figure 1); a catheter-based angiog...
متن کاملVeno-arterial extracorporeal membrane oxygenation support for severe cardiac failure in a pediatric patient with intracranial hemorrhage after spontaneous aneurysmatic rupture.
INTRODUCTION Extracorporeal life support in adult patients with extended intracranial hemorrhage is controversial. In pediatric patients, it has traditionally been considered a contraindication as systemic anticoagulation may worsen the hemorrhage and neurological outcome. CASE HISTORY We present a nine-year-old female patient who was admitted with extended intracranial hemorrhage after spont...
متن کاملTeaching NeuroImages: Spontaneous resolution of a giant intracranial arachnoid cyst.
A giant arachnoid cyst (63 83 11 cm) of left middle cranial fossa was found incidentally in a 3-year-old boy with head MRI examination for increased head circumference (figure 1). Surgical intervention was refused by his parents since the boy was asymptomatic, and the patient was followed up. Repeated MRI performed 2 years later revealed disappearance of the cyst, and the compressed brain tissu...
متن کاملConsequences of Intracranial Hemorrhage and Its Effective Factors
Background and purpose: Stroke is the third leading cause of death and the most common debilitating neurological disease. Intracranial Hemorrhage (ICH) is the second most prevalent form of stroke after ischemia. The aim of this study was to investigate the consequences of ICH and its effective factors. Materials and methods: In this prospective cross-sectional study, 288 patients with non-tra...
