resolved. The dorsum of his right foot had only small, red macules, and erythema in the pretibial region had resolved. BCG vaccine has been used for decades for the prevention of tuberculosis. More recently, BCG has been used as immunotherapy for various tumors. Importantly, the dosing of BCG is much higher when given as cancer immunotherapy [5]. As a result, BCG immunotherapy is often complicated by systemic side effects. Although BCG has been recovered from blood immediately after subcutaneous injection [6], attempts at isolating the organism from skin specimens from patients with chronic cutaneous changes have been futile [7]. Therefore, the cause of the cutaneous granulomatous lesions has remained controversial. Even though some investigators have postulated a hypersensitivity reaction to BCG as the underlying etiology, others have pointed to the rapid response of patients to antituberculosis treatment as evidence for direct invasion [5]. To our knowledge, we report the first case of culture-proven BCG infection of a chronic cutaneous lesion. When combined with the granulomatous changes seen during pathological evaluation and an excellent clinical response to antituberculosis therapy, it appears that direct BCG infection is the etiology of at least some of the postimmunization cutaneous abnormalities in our patient. Septic metastatic endophthalmitis complicating Klebsiella pneumoniae liver abscess in a non-diabetic Chinese man.
Septic metastatic endophthalmitis is a rare but serious disease. Endophthalmitis arising from Klebsiella pneumoniae liver abscess has been reported with diabetes mellitus as a major associated condition, but is rarely seen in patients without diabetes. A non-diabetic patient with liver abscess complained of right eye discomfort and floaters 3 days after admission. Both blood and liver aspirate ...
متن کاملIdentification of a major cluster of Klebsiella pneumoniae isolates from patients with liver abscess in Taiwan.
Klebsiella pneumoniae has emerged as the leading liver abscess pathogen in Taiwan, with the percentage rising from 30% in the 1980s to over 80% in the 1990s. Most of the patients with K. pneumoniae liver abscess are diabetic and without biliary tract disease. Some patients develop serious extrahepatic complications such as endophthalmitis, meningitis, lung abscess, and necrotizing fasciitis. Pu...
متن کاملAdult Klebsiella pneumoniae meningitis in Taiwan: an overview.
Klebsiella (K.) pneumoniae infections, including adult bacterial meningitis (ABM), are a distinct syndrome in Taiwan, which may consist of diabetes mellitus and multiple septic metastatic lesions such as liver abscess, endophthalmitis, and focal suppuration of other internal organs. In this review article, the authors will discuss the protean clinical manifestations and the complexity of the cl...

Bacterial endophthalmitis is endogenous in 2-6% of cases and is frequently misdiagnosed initially. Klebsiella pneumoniae is being increasingly recognised as an aggressive causative organism, and it is particularly prevalent in Asian populations. We describe the case of a 71-year-old female of Southeast Asian origin with type 2 diabetes mellitus who presented with visual loss secondary to bacter...

متن کاملA lack of ongoing diabetes is an important factor in preserving eyes from late or suboptimally treated endogenous endophthalmitis secondary to Klebsiella pneumoniae liver abscess
PURPOSE The purpose of this study is to identify the possible factors for preserving the eyes after late or suboptimally treated endogenous endophthalmitis secondary to Klebsiella pneumoniae (KP) liver abscess. METHODS A retrospective chart review was conducted for patients admitted with KP liver abscess from January 1991 to June 2012. RESULTS Six hundred and ninety-three patients with KP l...
