Oral Cobalamin (Vitamin B12) Therapy: From Empiricism and Personal Experience to Evidence Based Medicine and Perspective of Recommendations and Guideline

چکیده
In most countries, the classic treatment for cobalamin deficiency is based on parenteral administration as intramuscular injections in the form of cyanocobalamin and, more rarely, hydroxylor methylcobalamin [1]. In USA and UK, doses ranging from 100 to 1,000 μg per month (or every 2-3 months when hydroxocobalamin is given) are used for the duration of the patient’s life [2]. In France, treatment involves the administration of 1,000μg of cyanocobalamin per day for 1 week, followed by 1,000 μg per week for 1 month, followed by 1,000μg per month, again, normally for the remainder of the patient’s lifetime [3].

Effects of Vitamin B12 in Neonates and Young Infants
Vitamin B12 (cobalamin) is an essential coenzyme for nucleic acid synthesis. Animal protein is the major dietary source of vitamin B12. Deficiency of vitamin B12 leads to megaloblastic anemia, degeneration of the brain, spinal cord, peripheral nerves, and abnormalities of epithelial tissues. Two factors are necessary for the cure of megaloblastic anemia: one in food (extrinsic factor) and one i...

متن کاملCobalamin Deficiency in Elderly Patients: A Personal View
Cobalamin (vitamin B12) deficiency is particularly common in the elderly (>65 years of age) but is often unrecognized because its clinical manifestations are subtle; however, they are also potentially serious, particularly from a neuropsychiatric and hematological perspective. In the elderly, the main causes of cobalamin deficiency are pernicious anemia and food-cobalamin malabsorption. Food-co...
