Uric acid reduction rectifies prehypertension in obese adolescents.

چکیده
Epidemiologic studies, animal models, and preliminary clinical trials in children implicate uric acid in the development of essential hypertension. Controversy remains as to whether the observations indicate a general mechanism or a surrogate phenomenon. We sought to determine whether uric acid is a causative mediator of increased blood pressure (BP) and impaired vascular compliance. We report a randomized, double-blinded, placebo-controlled trial comparing 2 mechanisms of urate reduction with placebo in prehypertensive, obese, adolescents, aged 11 to 17 years. Subjects were randomized to the xanthine oxidase inhibitor, allopurinol, uricosuric, probenecid, or placebo. Subjects treated with urate-lowering therapy experienced a highly significant reduction in BP. In clinic systolic BP fell 10.2 mm Hg and diastolic BP fell 9.0 mm Hg in treated patients compared with a rise of 1.7 mm Hg and 1.6 mm Hg systolic and diastolic BP, respectively in patients on placebo. Urate-lowering therapy also resulted in significant reduction in systemic vascular resistance. These data indicate that, at least in adolescents with prehypertension, uric acid causes increased BP that can be mitigated by urate lowering therapy.

The Role of Uric Acid in Hypertension of Adolescents, Prehypertension and Salt Sensitivity of Blood Pressure
Uric acid is the end product of purine metabolism. Metabolic disorders of uric acid are associated with many disease states. Substantial evidence suggests the possible role of uric acid as a mediator of high blood pressure. Elevated uric acid is closely associated with new onset essential hypertension in adolescents and prehypertension; and urate-lowering agents can significantly improve these ...
متن کاملUric acid indicates a high cardiovascular risk profile but is not closely associated with insulin resistance in obese adolescents.
W e comment on a recent study by Dehghan et al. (1), who identified high serum uric acid as an independent risk factor for type 2 diabetes in adults. We investigated serum levels of uric acid in a large cohort of obese adolescents (n 355; mean SD age 13.1 4.1 years and age range 10–22 years) and normal-weight control subjects (n 200) of similar age and sex distribution (2). Uric acid levels wer...
متن کاملAssociation between perfluoroalkyl acids and kidney function in a cross-sectional study of adolescents.
BACKGROUND Perfluoroalkyl acids are synthetic compounds widely used in industrial and commercial applications. Laboratory studies suggest that these persistent and bioaccumulative chemicals produce oxidant stress and damage glomerular endothelial cells, raising concern regarding the impact of these compounds on renal function. METHODS We performed cross-sectional analyses of data 1960 partici...

Results Increased blood pressure occurred in 32% of all samples with 27% of prehypertension and 5% hypertension. Dyslipidemia, cholesterol levels, high LDL and triglycerides and low HDL levels found respectively 40%, 41.3%, 25% and 43.8% of the total sample. Obese adolescents with increased blood pressure have high triglyceride levels compared with obese adolescents with normal blood pressure, ...
متن کاملThe role of uric acid in the insulin resistance in children and adolescents with obesity
OBJECTIVE To investigate the association between serum uric acid levels and insulin resistance in children and adolescents with obesity. METHODS Cross-sectional study with 245 children and adolescents (134 obese and 111 controls), aged 8 to 18 years. The anthropometric variables (weight, height and waist circumference), blood pressure and biochemical parameters were collected. The clinical ch...
