چکیده

Vitamin D among family members as pet keeping may be correlated with lifestyles involving increased outdoor exposure, such as dog walking. Prenatal vitamin D inadequacy has been hypothesized as a risk factor for pediatric atopy and asthma. Methods: To investigate potential relationships between household pet exposure and cord blood vitamin D concentrations, we analyzed information from a large, geographically-based, general risk birth cohort. Household pets were assessed during pregnancy and serum level of 25 (OH)D (25hydroxyvitamin D) in cord blood was used as the measure of vitamin D and a marker of maternal level. Because of notable differences in vitamin D concentrations between African Americans and Whites, analyses were stratified by race. Results: A total of 1055 newborns were included in the study: 62.4% were African Americans and 49.4% were female. For Whites, but not African Americans, having no pet compared to 1 or .1 pet during pregnancy was associated with lower cord blood vitamin D concentrations (37.7, 45.2, 47.0 nmol/L, respectively, P 1⁄4 0.001). Considering type of pet, the relationship for no pet compared to 1 or .1 dog (37.7, 46.1, 49.9 nmol/L, respectively, P 1⁄4 0.001) was similar to that for no pet versus 1 or .1 cat (37.7, 43.0, 46.5 nmol/L, respectively, P 1⁄4 0.065). Conclusions: In a large ethnically diverse cohort of newborns, the presence of a pet in the home during the prenatal time period was associated with higher cord blood vitamin D, but only among Whites. This racial difference may reflect an impact on pet owner behavior resulting in increased outdoor exposure that is limited to lighter skinned individuals. However, as the effect doesn’t vary by cats versus dogs, differences by race in factors correlated with pet ownership or variations in pet keeping styles may be more important. Vitamin D should be considered in studies of pets and atopic conditions.

Background Inhaled corticosteroids are indicated in children who have more than mild persistent asthma. Fluticasone propionate is a newer corticosteroid agent with more potency compared with previous generations. However, still few dose ranging studies have investigated optimal dosing of inhaled corticosteroids particularly in children regarding the tolerability and safety of the drug. The prim...

Inhaled ciclesonide versus inhaled budesonide or inhaled beclomethasone or inhaled fluticasone for chronic asthma in adults: a systematic review

BACKGROUND Ciclesonide is a new inhaled corticosteroids licensed for the prophylactic treatment of persistent asthma in adults. Currently beclomethasone dipropionate, budesonide and fluticasone propionate are the most commonly prescribed inhaled corticosteroids for the treatment of asthma but there has been no systematic review comparing the effectiveness and safety ciclesonide to these agents....

Comparison of effectiveness in ciclesonide and fluticasone propionate on small airway function in mild asthma.

BACKGROUND Inhaled corticosteroids (ICS) are the mainstay of asthma treatment, but conventional ICS may have limited effectiveness in inflammation and patency of small airways. Ciclesonide is delivered and deposited in the peripheral region of the lung as a small particle corticosteroid. The aim of the study is to compare the effects of ciclesonide with fluticasone propionate on small airway fu...

Profile of ciclesonide for the maintenance treatment of asthma

Ciclesonide is a nonhalogenated synthetic inhaled corticosteroid (ICS) that has been approved by the US Food and Drug Administration for the treatment of all severities of persistent asthma. It is available as a hydrofluroalkane pressurized metered-dose inhaler in two strengths, 80 mcg/activation and 160 mcg/activation, with the recommenced dosage being two inhalations twice-daily. It is a prod...

Efficacy of Low-Dose Ciclesonide and Fluticasone Propionate for Mild to Moderate Persistent Asthma

BACKGROUND The aim of this study was to compare the efficacy of ciclesonide (80 mg/day) and fluticasone propionate (200 mg/day) for mild to moderate persistent asthma. MATERIALS AND METHODS Female and male patients older than 12 years with a history of persistent bronchial asthma for at least 6 months were enrolled. Patients were eligible to enter into a 2-week run-in period before randomizat...

Ciclesonide for the treatment of asthma

Ciclesonide is a nonhalogenated corticosteroid that is converted to its clinically active metabolite, desisobutyryl-ciclesonide, by esterases in the airways. Pharmacodynamic studies have shown that inhaled ciclesonide has potent antiinflammatory activity in patients with asthma, and does not appear to have clinically relevant systemic effects, even at high doses. It is highly protein-bound and ...
