چکیده

Coronary angiography (CA) to screen for the presence of coronary artery disease (CAD) is routinely performed before elective heart valve surgery. Its use is liberally advocated across a wide spectrum of patients by the writing committees for the treatment of patients with valvular heart disease (VHD) of the American College of Cardiology/ American Heart Association and the European Society of Cardiology.1,2 Class I indications, all with level of evidence C, include men 35 to 40 years or older, postmenopausal women, and premenopausal women with coronary risk factors, in addition to the more traditional considerations of suspected myocardial ischemia, left ventricular systolic dysfunction, and any history or other signs of CAD. These recommendations are predicted on 2 major factors; namely, that clinical, ECG, echocardiographic, and radionuclide markers are less specific for CAD in the setting of severe VHD and that perioperative and long-term outcomes are improved in patients with significant CAD undergoing combined valve and coronary bypass surgery compared with patients in whom revascularization is not performed. The evidence base for this latter claim derives primarily from older, single-center observational studies in patients with aortic stenosis (AS) and CAD.3,4 The more recent application of “hybrid” percutaneous coronary intervention with less invasive valvular heart surgery is gaining increasing acceptance,5 though there remains concern regarding the hazards and timing of dual antiplatelet therapy and institutional practices vary widely.

Coronary computed tomography angiography detection of short- and long-term outcomes after heart valve surgery with high risk cardiovascular patients

Coronary computed tomography angiography (CCTA) is a promising alternative technique to detect significant coronary artery lesions in high-risk cardiovascular patients with left ventricular dysfunction (left ventricular ejection fractions < 40%) referred for elective valve surgery, while little research about the use of CCTA to detect the outcomes of heart valve surgery was performed. Forty-six...

Quantification of coronary artery stenosis with 16-slice MSCT in patients before CABG surgery: comparison to standard invasive coronary angiography.

OBJECTIVE Invasive, selective coronary angiography is the gold standard for evaluation of coronary artery disease (CAD) and degree of stenosis. The purpose of this study was to compare 3-dimensional (3D) reconstructed 16-slice multislice computed tomographic (MSCT) angiography and selective coronary angiography in patients before elective coronary artery bypass graft (CABG) procedure. METHODS...

Pre-operative computed tomography coronary angiography to detect significant coronary artery disease in patients referred for cardiac valve surgery.

OBJECTIVES We studied the diagnostic performance of 64-slice computed tomography coronary angiography (CTCA) to rule out or detect significant coronary stenosis in patients referred for valve surgery. BACKGROUND Invasive conventional coronary angiography (CCA) is recommended in most patients scheduled for valve surgery. METHODS During a 6-month period, 145 patients were prospectively identi...

Value and limitations of computed tomography in assessing aortocoronary bypass graft patency.

To determine the value of nondynamic computed tomography (CT) in assessing aortocoronary bypass graft patency, we studied 67 patients with 125 grafts by CT and by coronary angiography at close time intervals. CT scans were performed before and after one to three (average 1.98 +/- 0.65) 50-ml i.v. bolus injections of contrast material. Eighty-four of 92 grafts patent at angiography were also vis...

David procedure through an upper partial sternotomy.

A 47-year-old female patient presented with congestive heart failure [New York Heart Association (NYHA) Class III]. A coronary angiogram showed calcified coronary arteries without any significant stenosis. Echocardiography showed a tricuspid aortic valve with grade III aortic insufficiency. A computed tomography (CT) scan of the thorax showed an aneurysm of the aortic root and the ascending aor...
