چکیده
A 63-year-old woman had gone to the emergency room several times for recurring episodes of headaches, tachycardia with palpitations, anxiety and sweating in the context of hypertensive crisis. High levels of adrenaline, noradrenaline, metanephrine and normetanephrine were found in urine. RM detected a hypervascular 5 cm in size right adrenal mass in contact with the inferior cava vein compatible with pheochromocytoma, associated with a solid exophytic hypervascular hepatic lesion suggestive of metastases at the left liver lobe (LLL) and an hepatic subcapsular hemangioma at the right liver lobe (RLL). For a better characterization and staging, adrenal scintigraphy was recommended.1 Whole-body 131I-MIBG (Fig. 1) showed intense uptake in right adrenal mass concordant with the diagnosis of pheochromocytoma. There was no evidence of focal uptake at the suspicious liver metastases on planar or SPECT images. A later 99mTc-autologous red blood cell (RBC) scintigraphy showed typical imaging of hepatic hemangioma2 located at the LLL (Fig. 2). Due to the disagreement of morphological and functional images, the studies achieved were recovered from PACS and a multimodality 131I MIBG/MR and 99mTc-RBC/MR registration and fusion was performed. The suspicious liver metastases at the LLL according to RM evidenced a high red-cell content (Fig. 3A), while the possible angiomatous lesion at the RLL was negative for it at merged fusion images (Fig. 3B). The final conclusion of the nuclear medicine studies was a right pheochromocytoma without extraadrenal extension. The patient underwent a laparoscopic resection of the right adrenal gland. During the intervention, the surgeons visualized a superficial angioma at the LLL and a smooth subcapsular lesion with benign aspect at the RLL. Pathological analysis revealed a 39 g and 5 cm × 3 cm × 3 cm pheochromocytoma with more than 4 tumor morphological criteria for potentially malignant biological behavior. One year after

Adrenocortical Carcinoma Mimicking Pheochromocytoma: A case Report and Review of Literature
Adrenocortical carcinoma (ACC) is an uncommon malignancy originating from cortex of adrenal gland. The most common pitfall in diagnosis of ACC is to distinguish it from pheochromocytoma. Here we report a 62-year-old hypertensive man with presentation of dyspnea. The laboratory data showed an increase in urine cortisol and renin with a mild increase in aldosterone, but decrease in adrenocorticot...
متن کاملINSULIN REQUIRING DIABETES MELLITUS AS A FEATURE Of PHEOCHROMOCYTOMA: REPORT OF A CASE
We report the case of a 65 year old man with recent onset of insulin requiring diabetes mellitus, frequent attacks of anginal chest pain, paroxysmal hypertension poorly controlled with three medications, hyperlipidemia, and mild renal insufficiency. The patient was found to have pheochromocytoma of the left adrenal gland, resection of which resulted in total resolution of diabetes, hyperte...
متن کاملMultimodality imaging of a cardiac paraganglioma
Paragangliomas are rare neuroendocrine tumors arising from the ganglia of the sympathetic or parasympathetic nervous system. Less than 160 cases of intrapericardial or intracardiac paragangliomas have been reported in the English language peer-reviewed medical literature. Here, we report a case of intrapericardial paraganglioma, which illustrates some typical multimodality imaging features of t...
متن کاملMyocardial Perfusion SPECT Imaging in Dextrocardia with Situs Inversus: A Case Report
Dextrocardia is a cardiac positional anomaly in which the heart is located in the right hemithorax with its base-to-apex axis directed to the right and caudad. Situs inversus is an autosomal recessive disorder that causes organs in the chest and abdomen to be positioned in a mirror image from their normal position. Dextrocardia may occur in isolation or as part of situs inversus. Similarly, sit...
متن کاملPHEOCHROMOCYTOMA OF URINARY BLADDER: REPORT OF A CASE AND BRIEF REVIEW OF THE LITERATURE
A case of pheochromocytoma of the bladder is presented with a r eview of the literature. We discuss general management principles of bladder pheochromocytoma. Successful diagnosis and treatment of this lesion requires a high index of suspicion based upon the patient's symptom complex, allowing preoperative preparation prior to surgical treatment.
