Misplaced left internal jugular venous catheter with an exceptional location

چکیده
Large numbers of central venous catheters (CVCs) are placed each year in the intensive care units and misplacement occurs frequently. Many critically ill patients require central venous catheterization for multiple and varied reasons. Internal jugular vein (IJV) catheter is one of the most frequent central venous catheters in intensive care units not only in Africa but all over the word. The right position of the catheter should be always verified by chest X ray after each catheterization even when it is ultrasound-guided. Acquired abnormalities such as stenosis or thrombosis of the central veins can be problematic. Catheters can also be misplaced outside veins in a patient with otherwise normal anatomy with potentially disastrous consequences. We report in this work an exceptional image of chest X ray showing an extremely rare misplacement of an internal jugular vein central catheter. This occurred in a 73-yearsold man admitted to our intensive care unit for an acute respiratory failure due to right pulmonary complex contusions due to a traffic accident which required intubation and mechanical ventilation. The chest X ray presented in this work shows an axillary location of the central catheter. It was documented that the catheter was inserted into the left axillary vein. Interestingly, there was no problem for infusion of fluids via this catheter. However, we removed the catheter without further complications and IJV cannulation was performed successfully via left. With regard to misplaced CVCs, in the short term, a useful aide memoir is: "if in doubt, don't take it out".

Misplaced subclavian central venous catheter
Percutaneous Central Venous Catheter (CVC) insertion using internal jugular and Subclavian veins routes is common procedure for all intensive care admitted patients and some patients in the ward as demand arises in central and referral hospitals of Botswana. This is a case report of a patient on whom a third attempt of re-inserting a CVC for fluid and total parenteral nutrition (TPN) was made. ...
متن کاملInternal jugular vein occlusion test for rapid diagnosis of misplaced subclavian vein catheter into the internal jugular vein.
BACKGROUND During subclavian vein catheterization, the most common misplacement of the catheter is cephalad, into the ipsilateral internal jugular vein (IJV). This can be detected by chest radiography. However, after any repositioning of the catheter, subsequent chest radiography is required. In an effort to simplify the detection of a misplaced subclavian vein catheter, the authors assessed a ...
متن کاملMalposition of central venous catheter in the left internal jugular vein--a case report.
Malposition of central venous catheter is a complication of central venous catheterization. A case of left internal jugular catheterization via left external jugular vein is reported. Details of the procedure are described and the literature is reviewed for similar malpositions.
متن کاملA left jugular vein catheter for hemodialysis malpositioned in right brachiocephalic vein.
A 69-year-old woman with obstructive nephropathy required emergent hemodialysis. The initial placement of a catheter into the right jugular vein was not successful as it was not possible to establish adequate blood flow. The second catheter introduced into the right subclavian vein failed too. Therefore, there was a need to place a catheter in the left jugular vein. However, chest radiography s...
متن کاملThe malposition of a central venous catheter through a sheath introducer via the left internal jugular vein
RATIONALE The misplacement of central venous catheter (CVC) can occur more often at the left jugular vein than the right side due to anatomic differences. And many of the previously reported cases are about catheter misplacement resulting from vessel penetration associated with guidewire. This case differs that the catheter itself through the sheath introducer can cause venous injury that may l...
