Determinants of antiarrhythmic drug efficacy for ventricular tachyarrhythmias using ambulatory monitoring and electrophysiological techniques.

چکیده
Sustained ventricular tachycardia and ventricular fibrillation are life-threatening arrhythmias with high rates of recurrence.1'2 Empiric antiarrhythmic drug therapy has a low success rate for preventing recurrences of these arrhythmias.3 Therefore, the choice of drug therapy has centered around two methodologies to predict drug efficacy: programmed ventricular stimulation with serial drug testing and suppression of ambient complex ventricular ectopy and nonsustained ventricular tachycardia detected by ambulatory monitoring recording and exercise stress testing. There is evidence that both approaches can successfully identify appropriate prophylactic antiarrhythmic drug therapy for sustained ventricular tachyarrhythmias.3-6 The study reported in this issue of Circulation7 was designed to compare the effectiveness of these two methodologies in determining antiarrhythmic drug treatment for such arrhythmias and to identify any clinical characteristic that can help predict drug efficacy in the context of these different methodologies.

متن کاملDeterminants of predicted efficacy of antiarrhythmic drugs in the electrophysiologic study versus electrocardiographic monitoring trial. The ESVEM Investigators.
BACKGROUND The Electrophysiologic Study Versus Electrocardiographic Monitoring (ESVEM) study was designed to compare the accuracy of predictions of antiarrhythmic drug efficacy made by electrophysiological study (EPS) with those made by Holter monitoring (HM) combined with exercise testing. The present study describes the baseline characteristics and the response to drug efficacy tests of 486 r...

متن کاملSuppression of sustained ventricular tachyarrhythmias: a comparison of d,l-sotalol with no antiarrhythmic drug treatment.
OBJECTIVES This study evaluates the clinical efficacy of d,l-sotalol in patients with sustained ventricular tachyarrhythmias. BACKGROUND D,l-sotalol is an important antiarrhythmic agent to prevent recurrences of sustained ventricular tachyarrhythmias (VT/VF). However, evidence is lacking that an antiarrhythmic agent like d,l-sotalol can reduce the incidence of sustained ventricular tachyarrhy...
متن کاملA new method for evaluating antiarrhythmic drug efficacy.
To develop standards for distinguishing antiarrhythmic drug effect from spontaneous variability of premature ventricular complexes (PVCs), 21 males (mean age 56 +/- 8 years) with chronic ischemic heart disease and PVCs underwent symptom-limited treadmill exercise testing and 24-hour ambulatory monitoring before and after 2 weeks of placebo medication. Linear regression analysis was used to desc...
