Successful robotic-assisted division of symptomatic vascular ring.

چکیده
A 61-year-old woman presented with the primary complaints of chest heaviness and dysphagia over the past 10 years. Extensive work-up included a computerized tomographic angiography that revealed a right aortic arch with an aberrant left subclavian artery with a vascular ring completed by the fibrotic ligamentum arteriosum connecting the proximal aberrant left subclavian artery to the left pulmonary artery (Figure). The patient was scheduled for robotic-assisted division of the vascular ring and was positioned in an exaggerated right lateral decubitus with single right lung ventilation. Three robotic instrument ports were placed through 1-cm incisions: the camera port was introduced in the 5th intercostal space in the mid-axillary line, the right arm port was introduced through the 3rd intercostal space at the anterior axillary line, and the left arm port was positioned through the 6th intercostal space at the posterior axillary line. The ligamentum arteriosum was dissected and successfully divided (Video). The patient was extubated in the operating room and discharged after 3 days. At last follow-up, her symptoms had resolved.

Abnormal bisubclavian trunk arising from the aortic arch determined by cadaver dissection of a native dog: A case report
Congenital anomalies of the great thoracic vessels have been reported in 20% of dogs and cats. In some cases, the vascular ring anomalies remain unrecognized throughout the lifetime of an animal. This report describes a case with an unusual vascular ring anomaly (VRA) that was detected during dissection on a cadaver of an approximately two-year-old male native mixed breed dog. No history of the...
متن کاملAsymptomatic right circumflex aortic arch associated with ventricular septal defect and biscuspid aortic valve.
INTRODUCTION Anomalies of the aortic arch are frequent congenital malformations, which rarely form partial or complete vascular rings. A rare form of vascular ring is the encircling, or circumflex, aortic arch. CASE REPORT A 19-month-old boy, with no respiratory symptoms, was referred for ventricular septal defect (VSD) repair. Cardiac magnetic resonance imaging and echocardiography confirmed...
متن کاملRobotic-Assisted Repair of Iatrogenic Ureteral Ligation Following Robotic-Assisted Hysterectomy
BACKGROUND AND OBJECTIVES Ureteral injuries, while rare, do occur during gynecologic procedures. The expansion of laparoscopic and robotic pelvic surgical procedures increases the risk of ureteral injury from these procedures and suggests a role for minimally invasive approaches to the delayed repair of ureteral injuries. We present, to our knowledge, the first case of delayed robotic-assisted ...
متن کاملRight sided heart evaluation after successful mitral valve replacement.
Introduction: It is well-documented that right-sided heart dysfunction and significant tricuspid valve regurgitation (TVR) have adverse effects on patient outcomes after left-sided heart valve surgery. Therefore, the evaluation of right ventriclular (RV) function and TR severity in patients who had undergone mitral valve replacement (MVR), associated with/without concomitant su...
متن کاملCeliac artery compression syndrome: successful utilization of robotic-assisted laparoscopic approach.
Median arcuate ligament (MAL) syndrome, also known as the celiac axis compression syndrome (CACS) is rare, and a topic of ongoing academic controversy. CACS is a diagnosis of exclusion, characterized by the clinical triad of postprandial abdominal pain, weight loss, and vomiting. The classic management of CACS involves the surgical division of the MAL fibers. We report successful treatment of a...
