The temporal endoscopic midface lift - centrofacial rejuvenation without facial scars
نویسنده
چکیده
Aim: The author describes a new endoscopic midface lifting technique using solely temporal access and evaluates its advantages and perspectives. Methods: This is a single-surgeon case study. Through a short temporal incision, dissection is performed along a single plane connecting the superficial surfaces of both the facial and temporal superficial musculoaponeurotic system (SMAS) layers. Subsequent midface dissection divides the malar fat pad into the deeper and superficial parts. A new concept of a combination high malar SMAS lift and internal skin flap anchorage that provides long-term stability is described. Results: The temporal endoscopic midface (TEM) lift technique resulted in good objective results with high patient satisfaction. The temporal access allowed proper vertical vector correction of the sagging centro-facial structures. The resulting scar was inconspicuous and hidden in the hair bearing skin, and its length was measured between 5-6 cm. A conversion to the conventional long facial scar approach was completely avoided. Conclusion: The TEM lift is a new and effective procedure for facial rejuvenation, especially the midface and cheek. The results illustrate the importance of internal anchorage of both the midface SMAS and skin flap. These are the keys to longlasting and pleasing results. The procedure is best suited for younger men and women with little or no neck skin laxity.

IMPORTANCE The value of this study is to evaluate outcomes of endoscopic forehead-midface-lift surgery. Many surgeons are reluctant to offer this procedure for fear of change in the shape and appearance of the eyelid. OBJECTIVE To objectively evaluate the change in lateral canthal position following endoscopic forehead-midface-lift surgery. DESIGN A retrospective review of consecutive patie...

The endoscopic midface lift procedure has evolved from experience with postreduction soft tissue repair after facial fracture fixation. The procedure elevates and repositions midface soft tissue, which descends with facial aging; as well, it can correct periorbital congenital abnormalities, such as exorbitism and lateral canthal displacement. The procedure has been refined by the senior author ...

Facial rejuvenation has largely focused on surgical procedures of the lower and upper one thirds of the face. Over the past 15 years, research focus on the midface has given aesthetic facial surgeons more tools to improve the signs of aging. The term midface has been used with various definitions, but includes the lower eyelid subunit beginning at the inferior border of the tarsal plate and che...

Standard face-lift techniques are excellent for the treatment of the jawline and neck. Treatment of the area between the lower eyelid and the corner of the mouth required the development of techniques in the intermediate lamella of the face. Alternative techniques of subperiosteal dissection by means of lower eyelid incisions were described with good aesthetic results but at the expense of incr...

The use of fiberendoscopic video-assisted technique in facial rejuvenation is one of the most advances in aesthetic plastic surgery of the face. It substitutes the coronal incision without the necessity of skin resection and allows a vertical reposition of the mobile soft tissue of the midface in indicated cases. It can easily be done through a small incision of the scalp just behind the corona...
