ACE inhibitors after myocardial infarction: patient selection or treatment for all?
نویسندگان
چکیده
It is now clear that angiotensin-converting enzyme (ACE) inhibitors have an important role in the management of patients after myocardial infarction. Several clinical trials have shown statistically significant reductions in mortality (SAVE, AIRE, GISSI-3, ISIS4).1-4 Some may argue that the weight of evidence favours wide, if not universal, use of ACE inhibitors among post-infarct patients. Does this argument withstand closer scrutiny? If not, what criteria should be used to identify which patients should be treated?

The comparative treatment effectiveness and safety of tissue versus non-tissue ace inhibitors among the elderly after acute myocardial infarction
Angiotensin Converting Enzyme (ACE) inhibitors are one of the recommended prevention therapy for patients with acute myocardial infarction (AMI) in the clinical guidelines. Two types (tissue and non-tissue) of ACE inhibitors are available with huge cost difference but the comparative treatment benefit and risk between them are unclear. The objective of this study was to investigate the comparat...
متن کاملMYOCARDIAL INFARCTION Use of Angiotensin-Converting Enzyme Inhibitors at Discharge in Patients With Acute Myocardial Infarction in the United States: Data From the National Registry of Myocardial Infarction
Objectives: This study was undertaken to examine recent trends in the use of angiotensin-converting enzyme (ACE) inhibitor therapy in patients discharged after acute myocardial infarction (AMI) and to identify clinical factors associated with ACE inhibitor prescribing patterns. Background: Clinical trials have demonstrated a significant mortality benefit in patients treated with ACE inhibitors ...
متن کاملAre we inhibited? Renal insufficiency should not preclude the use of ACE inhibitors for patients with myocardial infarction and depressed left ventricular function.
CONTEXT Angiotensin-converting enzyme (ACE) inhibitors have been shown to decrease mortality in patients with myocardial infarction and depressed left ventricular function, but physicians may be reluctant to prescribe ACE inhibitors to patients with concomitant renal insufficiency. OBJECTIVE To evaluate whether patients with depressed left ventricular ejection fraction following acute myocard...
متن کاملSecondary prevention: improving outcomes following myocardial infarction.
In the first five years of the 1990s, the role of angiotensin converting enzyme (ACE) inhibitors in the treatment of patients with myocardial infarction was investigated in a series of large controlled trials involving more than 100 000 patients (table 1). The early use of ACE inhibitors after acute myocardial infarction (MI) has been investigated in four trials (CONSENSUS II, GISSI 3, ISIS 4, ...
متن کاملACE inhibitors in acute myocardial infarction: patient selection and timing.
Angiotensin-converting enzyme inhibitors have earned their place along with aspirin, b-blockers, and thrombolytic agents as medical therapies proven to reduce mortality rates in acute myocardial infarction. The results of well-conducted, randomized, controlled clinical trials have been so consistent and so conclusive that the emphasis now shifts from research to implementation. Because the tria...
