Benefits of pharmacists’ management and intervention regarding anticoagulation therapy management. Pharmacists play an essential role in drug adherence and medication management, and this is especially true when it comes to anticoagulation therapy management. Warfarin is widely used to for anticoagulation therapy, but requires careful monitoring and dose adjustment. Since this topic was first addressed in the Fall 2010 issue of The Translator, the scope of practice for pharmacists has expanded, especially regarding therapy management with the advent of portable international normalized ratio (INR) measuring devices and patient self testing. This issue of The Translator highlights four different cases where pharmacists have used their expertise and knowledge to improve management of anticoagulation therapy:
