Evaluation of neurogenic control of blood pressure in hypertension with tetraethylammonium chloride and spinal anesthesia.

چکیده
The contribution of sympathetic constrictor impulses to arteriolar tone is now recognized as playing an important though sometimes variable role in the maintenance of increased peripheral resistance in some patients with hypertension. However, lack of a clear understanding of the exact nature of autonomic influences and of their importance in relation to other pressor factors has complicated the development and evaluation of therapeutic measures for hypertension which have as their aim the modification of the "neurogenic component." 3 Results obtained after surgical sympathectomy have seemed encouraging in certain patients, discouraging in others. It has become apparent that operative results are inconsistent and that the most striking inconsistency is the effect on the blood pressure. Difficult, too, is the choice of patients who may be expected to benefit from this type of treatment; no satisfactory method of selection has yet been suggested. This study was undertaken in an effort to evaluate the nature of autonomic influences on blood pressure level as indicated by observations and comparisons of the responses to autonomic blockade with tetraethylammonium chloride 4 (TEAC) and spinal anesthesia to high thoracic levels.

متن کاملNEUROGENIC HYPERTENSION: REPORT OF FIVE CASES TO HIGHLIGHT THE CONTROVERSY
The entity of "neurogenic hypertension" is defined as arterial hypertension caused explicitly by derangement of the intricate network of the central nervous system. Among 193 cases of cranial rhizopathies operated on between 1984 and 1995 at this center, 5 cases of established arterial hypertension with concomitant rhizopathies also underwent ventrolateral medullary decompression. The elev...
متن کاملEvaluation of neurogenic and humoral factors in blood pressure maintenance in normal and toxemic pregnancy using tetraethylammonium chloride.
Elevation of the arterial blood pressure is one of the most characteristic signs of toxemia of pregnancy. That this blood pressure elevation must reflect an increase in peripheral resistance in the toxemic patient is well recognized. More and more emphasis has been placed on the observation of vasoconstrictor phenomena in this illness so that arteriolar spasm is now thought to represent a frequ...

Aims and Background: Although controversial, many studies have shown effectiveness of colloid loading as a substitute for crystalloids on reducing the incidence of hypotension in spinal anesthesia. This study was conducted to compare the effects of three intravenous fluid regimens on hemodynamic changes following spinal anesthesia in cesarean section. The regimens included 6% Hydroxy ethyl star...
متن کاملComparison of blood pressure measurements on the upper and lower extremities during spinal anesthesia
Introduction: During spinal anesthesia, measurement of blood pressure is often obtained by an indirect method using an inflatable cuff on the upper arm. The relationship between the blood pressure measurements at the two sites in patients undergoing spinal anesthesia is unclear. This study was designed to evaluate the difference observed in systolic blood pressure (SBP) taken from the upp...
