The implementation of a blood conservation strategy in critical care on transfusion requirements

چکیده
Introduction Patients admitted to intensive care units (ICUs) are commonly affected by anaemia, which is contributed to by iatrogenic blood loss from diagnostic testing [1], with potential risks of increasing transfusion requirements and negative effect on outcomes [2]. Recently published guidelines make several recommendations for the management of anaemia and red blood cells (RBCs) transfusion in adult ICU patients [3].

Blood conservation devices in critical care: a narrative review
Anaemia is associated with inferior outcomes in critically ill patients. It is difficult to prevent and is treated commonly with the transfusion of packed red cells. However, transfusion to augment oxygen delivery has not been shown to consistently offer a survival advantage when the haemoglobin concentration exceeds 7 g/dL. Several studies point to inferior outcomes when patients are transfuse...

OBJECTIVE To describe blood conservation strategies for critically ill patients. DATA SOURCES By using a predefined strategy, we searched the electronic databases of Medline, EMBASE, CINAHL, the Cochrane database of systematic reviews, Cochrane central register of controlled trials, ACP Journal Club, Database of abstracts of reviews and effects, and HealthSTAR for descriptions and evaluations...
متن کاملThe use of a blood conservation device to reduce red blood cell transfusion requirements: a before and after study
INTRODUCTION Anaemia and the associated need for packed red blood cell (PRBC) transfusions are common in patients admitted to the intensive care unit (ICU). Among many causes, blood losses from repeated diagnostic tests are contributory. METHODS This is a before and after study in a medical ICU of a university hospital. We used a closed blood conservation device (Venous Arterial blood Managem...
متن کاملTransfusion trigger in critically ill patients: has the puzzle been completed?
In stable critically ill children, the adoption of a restrictive transfusion strategy based on a predefined hemoglobin threshold of 7 g/dl significantly decreased transfusion requirements without affecting outcome. These results strengthen previous observations made in volume resuscitated adults when a similar blood transfusion strategy was used. It also indirectly corroborates studies reportin...

Despite improvements in surgical and anesthetic techniques, total joint arthroplasty (TJA) is still associated with substantial blood loss and postoperative anemia (1). A considerable portion of patients with postoperative anemia require blood transfusion, which has been shown to negatively affect the outcome of TJA and predisposes patients to development of surgical site infection and peripr...
