چکیده

•We reviewed 185 PVC catheter ablation cases and identified 17 in which PVCs were successfully ablated in the aortic cusps.•The MDI measured 0.53 ± 0.10 (median 0.50, IQR 0.47-0.60).•In all (100%) cases MDI was > 0.40 and in 7 (41%) cases it was > 0.55.•Pseudo-delta wave was absent in all but one PVC•PVC origin in the aortic cusps should be considered along with the epicardium when MDI is in the vicinity of 0.55.

Cardiomyopathy Due to Nonsustained Ventricular Tachycardia Originating from the Aortic Sinus Cusp

We report a case of idiopathic nonsustained ventricular tachycardia (VT) originating from the aortic sinus cusp referred for presyncope and LV dysfunction and frequent premature ventricular complex with no response to 3 months anti-arrhythmic medication for heart failure and arrhythmia. She was then referred to us for frequent PVC's and runs of nonsustained VT. ECG recorded during the nonsustai...

Ventricular arrhythmias arising from the left ventricular outflow tract below the aortic sinus cusps: mapping and catheter ablation via transseptal approach and electrocardiographic characteristics.

BACKGROUND Ventricular arrhythmias (VAs) originating from the anterosuperior left ventricular outflow tract (LVOT) represent a challenging location for catheter ablation. This study investigates mapping and ablation of VA from anterosuperior LVOT via a transseptal approach. METHODS AND RESULTS This study included 27 patients with symptomatic VA, of which 13 patients had previous failed ablati...

Bundle branch reentry: a possible mechanism of ventricular tachycardia.

Electrophysiologic studies were performed in three patients suffering from attacks of paroxysmal tachycardia with wide QRS complexes. Two patients had atrioventricular dissociation. The arrhythmia could be initiated and terminated by premature ventricular stimulation in all three patients. One patient developed the arrhythmia after rapid atrial stimulation. In each subject, the QRS complexes du...

Interesting response of narrow QRS tachycardia to premature atrial complex. What is the mechanism?

Fig.1 shows a premature atrial complex (PAC) introduced during a narrow QRS tachycardia. What is the response and what is the interpretation? The tachycardia is a regular RBBB tachycardia with cycle length of 440 ms and ventriculo-atrial interval of 30 ms. There is near simultaneous activation of atrium and ventricle during tachycardia. Presence of His deflection prior to the QRS makes ventricu...
