Endovascular Management of Wide-Necked Aneurysms

چکیده

Endovascular management of intracranial aneurysms using detachable platinum coils (Guglielmi detachable coils, Target Therapeutics, Fremont, CA) was introduced in 1990 by Guido Guglielmi, an Italian neurosurgeon. Working in collaboration with Vinuela and others at the University of California at Los Angeles, along with Target Therapeutics, endovascular obliteration of aneurysms became a common practice. Many studies have shown equivalent or improved shortand medium-term outcomes when comparing matched groups of patients undergoing surgical and endovascular therapy. Despite advances, however, the treatment of wide-necked aneurysms (aneurysms with a fundus-to-neck ratio less than 2) remains problematic. Results from early studies using single-dimensional coils alone demonstrated 72% to 80% complete occlusion when the ratio was greater than 2, and 53% when it was lower. Inability to keep coils within these lesions, the posttreatment presence of residual aneurysm, and the occurrence of delayed aneurysm recanalization led investigators to seek new ways to embolize these lesions safely and effectively. This article focuses on techniques and devices aimed at treating widenecked aneurysms.

A novel endovascular treatment of a wide-necked basilar apex aneurysm by using a Y-configuration, double-stent technique.

Wide-necked bifurcation aneurysms remain a formidable challenge to the neuroendovascular surgeon. A 36-year-old woman with a wide-necked basilar bifurcation aneurysm was unsuccessfully treated by endovascular methods, despite the use of the balloon-remodeling technique. Successful coiling was ultimately achieved by use of a Y-configuration double stent-assisted technique. This novel method of u...

Endovascular occlusion of intracranial aneurysms with electrically detachable coils: correlation of aneurysm neck size and treatment results.

PURPOSE To devise a method to measure aneurysm neck size on angiographic films, and to correlate the sizes obtained with the extent of endovascular aneurysm occlusion, performed with electrically detachable coils. METHODS The angiograms of 79 intracranial aneurysms treated by endovascular occlusion using electrically detachable coils were retrospectively analyzed. A method using the average r...

Ruptured carotid artery aneurysms of the ophthalmic (C6) segment: clinical and angiographic long term follow-up of a multidisciplinary management strategy.

BACKGROUND The management of ruptured C6 aneurysms remains controversial. Detailed long-term outcome data are still lacking. Thus the present study provided a detailed long term follow-up for a multidisciplinary approach combining microsurgical clipping, endovascular embolisation and parent artery occlusion with/without bypass protection. METHODS In our single centre analysis of 64 consecutiv...

Single-center experience in the endovascular treatment of wide-necked intracranial aneurysms with a bridging intra-/extra-aneurysm implant (pCONus)

PURPOSE To retrospectively evaluate the safety and efficacy of the endovascular treatment of wide-necked intracranial aneurysms assisted by a novel intra-/extra-aneurysm stent-like implant (pCONus). METHODS Initial and follow-up angiographic and clinical results are presented of 25 patients with 25 unruptured and ruptured wide-necked intracranial aneurysms treated by reconstruction of the ane...

Key perspectives on Woven EndoBridge device for wide-necked bifurcation aneurysms, endoscopic endonasal clipping of intracranial aneurysms, retrosigmoid versus translabyrinthine approaches for acoustic neuromas, and impact of local intraoperative steroid administration on postoperative dysphagia following anterior cervical discectomy and fusion

Despite the great overall success of endovascular therapy for treatment of both ruptured and unruptured aneurysms, the endovascular treatment of wide‐necked bifurcation aneurysms remains challenging. The WEB device was created as a novel treatment for these aneurysms, for which balloon‐ or stent‐assisted coiling, and surgery remain as alternatives, although often challenging. The WEBCAST Key pe...
