Prophylactic Strategy for Peritoneal Metastasis from Gastric Cancer

چکیده
The prognosis of advanced gastric cancer patients with peritoneal metastasis is extremely poor. We advocate extensive intraoperative peritoneal lavage (EIPL) as a useful and practical adjuvant surgical technique for gastric cancer patients who are likely to suffer from peritoneal recurrence. In this article, we review the ability of EIPL to prevent peritoneal recurrence in patients with peritoneal free cancer cells, but not overt peritoneal metastasis (CY+/P-), and describe its potential as a prophylactic therapeutic strategy against peritoneal recurrence.

A Proposal of a Practical and Optimal Prophylactic Strategy for Peritoneal Recurrence
Peritoneal metastasis, which often arises in patients with advanced gastric cancer, is well known as a miserable and ill-fated disease. Once peritoneal metastasis is formed, it is extremely difficult to defeat. We advocated EIPL (extensive intraoperative peritoneal lavage) as a useful and practical adjuvant surgical technique for those gastric cancer patients who are likely to suffer from perit...
متن کاملStandard Prophylactic Strategy against Peritoneal Dissemination Metastasis in Gastric Cancer
Peritoneal carcinomatosis is the most frequent pattern of metastasis and recurrence in patients with gastric cancer, and the prognosis of those patients with peritoneal metastasis is extremely poor. Once peritoneal metastasis is formed, it is extremely difficult to overcome. EIPL (extensive intraoperative peritoneal lavage) is a quite useful and practical adjuvant surgical technique for the gas...

PURPOSE This aims of this study are to establish an ultra-rapid quantitative reverse transcription-PCR (RT-PCR) protocol that enables the diagnosis of i.p. cancer spread during operation, to reveal the mechanisms of peritoneal metastasis from non-serosa-invasive gastric carcinoma, and to evaluate the effect of the extensive intraoperative peritoneal lavage (EIPL) using the ultra-rapid quantitat...
متن کاملProphylactic gastrectomy in familial gastric cancer: case report and review of literature
Absract Gastric cancer is the fourth most common cancer and the second leading cause of cancer death. Most cases are sporadic and only 10% of patients, show familial clustering. Among these patients, 1 to 3 % have hereditary diffuse gastric cancer (HDGC), which is autosomal-dominant and present in younger ages. Mutations in Ecadherin gene CDH1 has been identified in 30 to 50% of patient...
متن کاملRegional but fatal: Intraperitoneal metastasis in gastric cancer
Peritoneal carcinomatosis appears to be the most common pattern of metastasis or recurrence and is associated with poor prognosis in gastric cancer patients. Many efforts have been made to improve the survival in patients with peritoneal metastasis. Hyperthermic intraperitoneal chemotherapy remains a widely accepted strategy in the treatment of peritoneal dissemination. Several phase II-III stu...
