چکیده

Pegaptanib (Eyetech Pharmaceuticals and Pfizer), which was approved by the FDA at the end of 2004, was the first anti-VEGF therapy for neovascular AMD (3). After approval of bevacizumab (Genentech/ Roche) for cancer therapy and in view of the suspected role of VEGF in wet AMD, systemic intravenous and, later, intravitreal bevacizumab began to be administered off-label to treat neovascular AMD (4). Ranibizumab (Genentech/ Novartis), a humanized monoclonal anti-Fab antibody, engineered to bind with high affinity to VEGF-A and inhibit all of its biologically active isoforms, was found to be an effective therapeutic agent for neovascular AMD. This was proven in two pivotal trials: the Minimally Classic/Occult Trial of the Anti-VEGF Antibody Ranibizumab in the Treatment of Neovascular AgeRelated Macular Degeneration (MARINA) and the AntiVEGF Antibody for the Treatment of Predominantly Classic Choroidal Neovascularization in Age-Related Macular Degeneration (ANCHOR) (5). Based on this evidence, ranibizumab was approved by the FDA in 2006 as therapy for the treatment of wet AMD (6).

Immediate loss of vision due to retinal pigment epithelial tear after anti-angiogenesis treatment of pigment epithelial detachment.

Intravitreal injection of ranibizumab, an antivascular endothelial growth factor (VEGF) drug, is currently the primary treatment for wet age-related macular degeneration (AMD) in the UK. Use of ranibizumab for the treatment of isolated pigment epithelial detachments (PEDs) without the presence of an occult choroidal neovascular membrane has not been studied in a randomized controlled fashion an...

Transpupilary thermotherapy of occult subfoveal choroidal neovascularization secondary to age related macular degeneration

Abstract Background: Transpupillary thermotherapy is a new treatment for subfoveal choroidal neovascularization which needs further evaluation. This study was aimed to evaluate the efficacy of transpupillary thermotherapy on regression of occult subfoveal choroidal neovascularization with or without pigment epithelial detachment in patients with age-related macular degeneration. Methods: In a ...

Conversion back to bevacizumab or ranibizumab for recurrent neovascular activity with aflibercept in age-related macular degeneration: a case series

BACKGROUND Neovascular age-related macular degeneration often requires chronic therapy with anti-VEGF agents, and patients with recurrent disease are challenging to manage. METHODS This retrospective case series evaluates patients who were switched from bevacizumab or ranibizumab to aflibercept and then back again because of recurrent fluid on optical coherence tomography (OCT) by reporting c...

Full thickness macular hole case after intravitreal aflibercept treatment

BACKGROUND The pathogenesis of macular hole formation is widely accepted as a tractional force at the vitreo-retinal interface in fovea. We report a case of macular hole after intravitreous aflibercept injection for age-related macular degeneration (AMD) associated with contraction of the retinal pigment epithelium (RPE) at the edge of a fibrovascular pigment epithelial detachment (PED). CASE...

One year results of anti-VEGF treatment in pigment epithelial detachment secondary to macular degeneration Resultados após um ano de tratamento anti-VEGF para o descolamento do epitélio pigmentado secundário à degeneração macular

INTRODUCTION Age-related macular degeneration (ARMD) is a very common cause of vision loss especially in elderly patients living in developed countries. ARMD is caused by the accumulation of drusen in the macula, which is the most important part of the retina, and this process ends in serious irreversible visual impairment. Pigment epithelial detachment (PED) may be seen in all stages of ARMD a...
