Deep inferior epigastric perforators do not correlate between sides of the body: the role for preoperative imaging.

چکیده
The deep inferior epigastric artery (DIEA) perforator (DIEP) flap has been widely described as a safe flap with reliable vascular anatomy. One of its limitations is potentially lengthy operating times during flap harvest, and many techniques have been reported to reduce operating times. One such technique is the preoperative selection of perforators with imaging, which has been shown to substantially reduce operating times. In the past, a technique utilised for perforator selection was to raise one hemi-abdominal wall purely for perforator identification, and to use the location of perforators as a predictor for the contralateral hemiabdominal wall. However, anatomical studies have highlighted substantial variability between DIEA perforators both within and between individuals, and the anatomical basis for such a technique has not been demonstrated in any anatomical studies. The current study thus sought to compare the abdominal wall perforators of each side of the body to confirm whether such a correlation between sides exists.

A Preoperative Marking Template for Deep Inferior Epigastric Artery Perforator Flap Perforators in Breast Reconstruction
Preoperative perforator marking for deep inferior epigastric artery perforator flaps is vital to the success of the procedure in breast reconstruction. Advances in imaging have facilitated accurate identification and preselection of potentially useful perforators. However, the reported imaging accuracy may be lost when preoperatively marking the patient, due to 'mapping errors', as this relies ...
متن کاملCASE REPORT Case Report and Review of the Literature: Deep Inferior Epigastric Perforator Flap for Breast Reconstruction After Abdominal Recontouring
OBJECTIVE The report herein presents a case of a 49-year-old woman with left breast cancer who presented seeking immediate autologous reconstruction. Surgical history included an abdominal hysterectomy and an abdominal contouring procedure. This is a first description of a deep inferior epigastric perforator flap after abdominal wall manipulation of this magnitude. METHODS Computed tomographi...
متن کاملMicrocirculatory Evaluation of the Abdominal Skin in Breast Reconstruction with Deep Inferior Epigastric Artery Perforator Flap
UNLABELLED No studies have assessed the perfusion of the undermined abdominal skin in breast reconstruction with deep inferior epigastric artery perforator flap. A greater understanding of the procedure's impact on the perfusion of the abdominal skin can be valuable in predicting areas susceptible to necrosis. METHODS Microcirculatory changes were monitored in the abdominal skin of 20 consecu...

OBJECTIVE To find out the most suitable perforator and cutaneous nerve for strategic design of the deep inferior epigastric perforator (DIEP) flap. MATERIAL AND METHOD The characteristics of the pedicles, perforators, intercostal nerves and the relationship between nerves and vessels in DIEP flaps were studied in 31 formalin-preserved cadavers. RESULTS Four hundred and five perforator vesse...
متن کاملEnhanced Preoperative Deep Inferior Epigastric Artery Perforator Flap Planning with a 3D-Printed Perforasome Template: Technique and Case Report
Optimizing preoperative planning is widely sought in deep inferior epigastric artery perforator (DIEP) flap surgery. One reason for this is that rates of fat necrosis remain relatively high (up to 35%), and that adjusting flap design by an improved understanding of individual perforasomes and perfusion characteristics may be useful in reducing the risk of fat necrosis. Imaging techniques have s...
