Sarcoidosis or sarcoid reaction?

چکیده
McNeill et al1 reported a patient whose sarcoidosis and lung cancer coexisted, raising the question of causality and, secondarily, of cause vs effect. The Ptolemaic (sarcocentric) viewpoint, advanced by Brincker and Wilbek,2 posits that the disease sarcoidosis can induce the development of solid neoplasms by an unspecified mechanism. The Copernican (oncocentric) viewpoint posits that sarcoidosis is an etiologically diverse syndrome,3 and that when systemic granulomas coexist with neoplasia, they constitute an immunologic response.4 The following considerations favor the Copernican viewpoint:

Sarcoidal Type Foreign Body Reaction: A Case Report
Sarcoidosis and foreign body reactions are common differential diagnoses of skin granuloma formation. We report a 40 year –old man with sarcoid type granulomas containing polarized foreign materials, and also bilateral hilar adenopathy. It appears that detection of foreign bodies in histopatologic studies of granulomas does not necessarily exclude the possibility of sarcoidosis.
متن کاملSarcoidosis and sarcoid-like reaction associated with pancreatic malignancy: are you able to read a riddle?
CONTEXT The sarcoidosis is an idiopathic multisystem inflammatory disease characterized by the presence of non-caseating granulomas in the affected organs. The clinical picture includes non-specific systemic symptoms and organ-specific symptoms, but it is frequently asymptomatic. Although not fully understood, a clear association between sarcoidosis and malignancies has been reported. In neopla...
متن کاملSarcoid reaction associated with Merkel cell carcinoma revealed by fluorodeoxyglucose positron emission tomography: a case report
Introduction: Although the association between cancer and sarcoidosis or sarcoid reaction is known, sarcoid reaction associated with Merkel cell carcinoma is rare. Case presentation: We report the case of a 57-year-old Japanese woman with Merkel cell carcinoma in the inguinal area associated with sarcoid reaction. Fluorodeoxyglucose positron emission tomography demonstrated elevated fluorodeoxy...
متن کاملEvaluation of Mediastinal Lymph Nodes in Sarcoidosis, Sarcoid Reaction, and Malignant Lymph Nodes Using CT and FDG-PET/CT
The aim of this study was to analyze the clinical, computed tomography (CT), and positron emission tomography (PET) findings of sarcoidosis, sarcoid reaction, and malignant lymph nodes (LNs) to the results of transbronchial LN aspiration and biopsy (TBNA).The TBNA results of mediastinal and hilar LNs of 152 patients in our hospital from July 2008 to March 2013 were retrospectively reviewed. Two...
متن کاملAdalimumab induced pulmonary sarcoid reaction
Sarcoidosis is a multisystem granulomatous inflammatory disease of unknown etiology. There is evidence that Tumor Necrosis Factor alpha (TNF-α) antagonists are useful in the treatment of advanced or refractory disease. However, sarcoidosis-like reaction has been reported with TNF-α blockade in other inflammatory conditions. Here we report a case of sarcoid-like reaction in a patient with psoria...
متن کاملAccumulation of common clonal T cells in multiple lesions of sarcoidosis.
BACKGROUND T cells recognizing as yet unknown antigens (Ags) are considered to play an important role in the development and perpetuation of the disease process of sarcoidosis. Several studies have shown that T cells that bear a limited T-cell receptor (TCR) repertoire may play an important role in this disorder. However, regarding variable (V) gene repertoire usage, the results differ among va...
