Amphotericin B in resistant visceral leishmaniasis.

چکیده
Sodium stibogluconate (SSG) in the dose of 20 mg/kg body weight daily intramuscularly for 20-30 days is recommended by WHO as the first line drug for Indian visceral leishmaniasis (VL). Relapses following therapy and unresponsiveness to the higher doses of SSG are still a problem in north Bihar. Serious toxicity of SSG including myocarditis, sudden death and renal failure have also been reported(l,2). Pentamidine isethionate is also extensively used as a second line drug for VL. The drug, however, has to be imported, is costly and toxic. Pentamidine induced diabetes mellitus and myocarditis leading to sudden deaths are not uncommon(3-6).

INTRODUCTION Current treatment options for visceral leishmaniasis (pentavalent antimony, amphotericin B, liposomal amphotericin B and mitelfosine) achieve long-term clinical cure in the majority of immunocompetent patients. Disease relapse is usually provoked by T-cell number or function impairment (corticosteroid or cytotoxic therapy, transplant recipients, advanced human immunodeficiency viru...
متن کاملUnusual case of resistance to amphotericin B in visceral leishmaniasis in a region in India where leishmaniasis is not endemic.
The case of a patient with visceral leishmaniasis showing inadequate response to amphotericin B from a region where leishmaniasis is not endemic is reported, with the Leishmania donovani isolate showing increased tolerance to amphotericin B in vivo. Four single nucleotide polymorphisms (SNPs) detected in the cysteine proteinase B gene resulted in changes to the deduced amino acid sequence: vali...

متن کاملChanging Pattern of Visceral Leishmaniasis, United Kingdom, 1985-2004
A 20-year (1985-2004) retrospective review of 39 patients with imported visceral leishmaniasis found that tourism to Mediterranean countries and HIV infection were associated with visceral leishmaniasis. Diagnosis was often delayed. Treatment with liposomal amphotericin B has improved prognosis. Visceral leishmaniasis should be made a reportable disease.
