Progestogens for Prevention of Preterm Birth Executive Summary

چکیده
Birth before completion of 37 weeks of pregnancy is considered preterm. These early births are associated with more than 85 percent of all perinatal morbidity and mortality and are the leading cause of infant mortality and long-term disability.1-2 Each year in the United States more than 475,000 infants are born preterm representing 12.5 percent of live births.3 Efforts to reduce preterm birth have been largely unsuccessful, with a 20 percent relative increase since 1990 in the proportion of births in the United States that are preterm.2

Progesterone and preterm birth prevention: translating clinical trials data into clinical practice.
OBJECTIVE We sought to provide evidence-based guidelines for using progestogens for the prevention of preterm birth (PTB). METHODS Relevant documents, in particular randomized trials, were identified using PubMed (US National Library of Medicine, 1983 through February 2012) publications, written in English, which evaluate the effectiveness of progestogens for prevention of PTB. Progestogens e...
متن کاملPrevention of preterm birth with vaginal progesterone or 17-alpha-hydroxyprogesterone caproate: a critical examination of efficacy and safety.
Progestogens are the first drugs to demonstrate reproducibly a reduction in the rate of early preterm birth. The efficacy and safety of progestogens are related to individual pharmacologic properties of each drug within this class of medication and characteristics of the population that is treated. The synthetic 17-hydroxyprogesterone caproate and natural progesterone have been studied with the...
متن کاملP-188: Vaginal Progesterone Effects for The Prevention of Preterm Birth and Neonatal Complications in Women at Increased Risk: A Randomized Placebo- Controlled Double-Blind Study
Background: The purpose of this study was to evaluate the effect of prophylactic vaginal progesterone in decreasing preterm birth rate and neonatal complications in a high-risk population. Materials and Methods: A randomized, double-blind, placebo- controlled study included 100 high-risk singleton pregnancies( prior preterm birth, short cervix, uterine malformations and large intramural uterine m...

2066 www.thelancet.com Vol 387 May 21, 2016 Preterm birth is the most common cause of neonatal morbidity and mortality globally, aff ecting about 15 million children every year. Of children born preterm, an estimated 2·4 million (15·6%) are born extremely preterm (before 28 weeks gestation) or very preterm (before 32 weeks gestation). Furthermore, some 1 million children every year die as a con...
متن کاملTrial protocol OPPTIMUM– Does progesterone prophylaxis for the prevention of preterm labour improve outcome?
BACKGROUND Preterm birth is a global problem, with a prevalence of 8 to 12% depending on location. Several large trials and systematic reviews have shown progestogens to be effective in preventing or delaying preterm birth in selected high risk women with a singleton pregnancy (including those with a short cervix or previous preterm birth). Although an improvement in short term neonatal outcome...
