Gastrocnemius-plasty for primary closure of compound injuries of the knee.

چکیده
Free skin grafting has become recognised as a valuable part of the primary treatment of traumatic skin defects. However, free skin grafts can be used only when the base of the wound is vascularised. Fascia and muscle are ideal recipient sites whereas covering of tendons, bones and open joints needs pedicle flaps carrying their own blood supply. When extensive skin loss has occurred around an open joint, it is not possible to close the joint with local skin plastic procedures. It is therefore usually necessary in such cases to employ skin flaps from neighbouring regions-for instance, abdominal flaps for the upper limbs or cross-leg flaps for the lower limbs. Extensive lesions of this type are seen in the lower limbs when an individual is dragged along the road surface by a motor vehicle. In two such cases, we have used a local muscle flap to cover the gaping knee instead of a flap from the opposite leg.

Distally-based vastus lateralis muscle flap: a salvage alternative to address complex soft tissue defects around the knee.
Repeated soft tissue injuries around the knee represent challenging clinical situations where functional prognosis of the joint is often poor, especially in the presence of total knee arthroplasty (TKA). When gastrocnemius muscle flap techniques have already been used and failed, therapeutic solutions become rare. The authors suggest a regional solution to salvage these cases by the transfer of...

Background: Hamstring muscles contracture is a major problem in the management of the cerebral palsy CP patient causing the crouch gait which is one of the most resistant conditions to treat. Methods: Twenty patients (40 knees) with spastic cerebral palsy diaplegia, between 7 – 12 years of age, presented to The National Institute of Neuromotor System of Egypt, from January 2010 to January 2011....
متن کاملOutcomes and Complications of Primary Closure of Wounds in the War Wounded Referred to a Hospital in Tehran in 2017
Background and Aim: In most studies, in the treatment of war wounds, debridement and delayed wound healing have been suggested and there has been no emphasis on initial wound closure. The aim of this study was to determine the outcomes and complications of primary closure of wounds in war-wounded referred to a hospital in Tehran, Iran. Methods: In this cross-sectional study, 102 war-wounded wit...
متن کاملS-plasty for pilonidal disease: modified primary closure reducing tension
PURPOSE S-plasty for pilonidal disease reduces the tension on the midline by distributing it diagonally and flattening the natal cleft. The aim of this study was to evaluate the outcomes of S-plasty on simple midline primary closure and the clinical features of pilonidal patients in a low incidence country. METHODS S-plasty was applied on 17 patients from July 2008 to October 2010. Data of th...
متن کاملComparison of Three Different Methods of Skin Closure in Anterior Midline Incisions of the Knee
Background: Few clinical trials have studied the functional and cosmetic outcomes of different closure tech- niques of surgical incisions. Skin wound and incision closure methods may influence the healing process and cos- metic outcome. The present study aims at comparing three different suture techniques of skin closure. Methods: In a clinical trial, sixty patients with anterior cruciate liga...
