The Absorption of Radioactive Vitamin B12 and the Secretion of Hydrochloric Acid in Patients with Atrophic Gastritis.

چکیده
it is now well known that chronic atrophic gastritis may occur in patients who do not have Addisonian pernicious anaemia, and that the histological appearance of the gastric mucosa in patients with and without pernicious anaemia may be indistinguishable. In fact there is no pathognomonic histological gastric lesion in pernicious anaemia (Magnus, 1958; Williams, Coghill, and Edwards, 1958). Furthermore defective absorption of vitamin B12 has been found in some patients with atrophic gastritis when there was no overt haematological evidence of Addisonian pernicious anaemia (Badenoch, 1954; Callender and Denborough, 1957; Mollin, Booth, and Baker, 1957; Siurala and Nyberg, 1957; Glass, Speer, Nieburgs, Ishimori, Jones, Baker, Schwartz, and Smith, 1960). However, some of these patients were either suffering from early Addisonian pernicious anaemia or were from families of patients with pernicious anaemia (Callender and Denborough, 1957; Siurala and Nyberg, 1957). In them factors other than gastric atrophy may contribute to diminished ability to secrete intrinsic factor (McIntyre, Hahn, Conley, and Glass, 1959). During the course of an investigation into the histological appearances of the gastric mucosa, patients were found with atrophic gastritis unassociated with clinical or haematological evidence of

TROPHIC GASTRITIS is a common occurrence in patients with autoimmune thyroid disease.1’2 The severity of the atrophic gastritis varies from achiorhydria with malabsorption of vitamin B12 ( Addisonian pernicious anemia ) ( P.A. ) , through achlorhydria with adequate vitamin B12 absorption, to minimal or normal secretion of gastric acid and intrinsic factor. It was therefore considered that the s...

Vitamin B12 deficiency is estimated to affect 10%-15% of people over the age of 60, and the laboratory diagnosis is usually based on low serum vitamin B12 levels or elevated serum methylmalonic acid and homocysteine levels. Although elderly people with low vitamin B12 status frequently lack the classical signs and symptoms of vitamin B12 deficiency, e.g. megaloblastic anemia, precise evaluation...
متن کاملIntrinsic factor secretion in gastric atrophy.
EDITORIAL SYNOPSIS The appearance of the gastric biopsy histologically has been correlated with assay of the intrinsic factor and acid content of the gastric juice, and also with vitamin B12 absorption studies. The loss of intrinsic factor parallels the loss of gastric secretory cells. The assay of intrinsic factor proved a better index of secretory capacity of the mucosa than either acid produ...
متن کاملThe effect of prednisolone on gastric mucosal histology, gastric secretion, and vitamin B 12 absorption in patients with pernicious anemia.
The effect of corticosteroids on the gastric mucosa in patients with pernicious anemia is of particular interest in view of the possibility that atrophic gastritis, the primary pathologic lesion in pernicious anemia, may result from destruction of gastric glands by an autoimmune process (1). The first observation which suggested that corticosteroids might enhance vitamin B12 absorption in patie...
