چکیده

We examined seizure outcome Cfree ,from seizures however minor vs. continuing seizures however infrequent) 5 years post-TLES for mesial temporal sclerosis in 82 consecutive patients, aged 8-43 years, who underwent pharmacologically activated sphenoidal EEG pre-operatively (12 with extracranial ictal monitoring (VT), 70 without). Fifty-six had suffered an early-childhood complicated convulsion (ECC+); 56 had strictly unilateral sphenoidal spikes (SphU). The 70 non-monitored patients were divided according to the presence/absence of ECCf and of SphU. Those seizure free were: 78%. ECCf, SphU (N = 37) + 14% Engel other 1-t 11; 60%, ECC+, not SphU (N = 10) + 30% Engel other I + 11; 47%, not ECC+, SphU (N = 15); 25%, not ECC+, not SphU (N = 8). VT patients had no better outcome in any group. Therefore, excellent outcome is achieved for most with both ECC+ and SphU making routine VT unnecessary. Videotelemetry may be indicated (often intracranially) for all without ECC+, and for some (usually only extracranially) with ECC+ but not SphU.

The value of long term EEG monitoring in children: A comparison of ambulatory EEG and video telemetry

PURPOSE Outpatient ambulatory EEG may be followed by inpatient video telemetry EEG when investigating children for possible seizures and for classification of epilepsy. We investigated the value of ambulatory EEG and subsequent video telemetry recording in our centre. METHOD The departmental EEG database was interrogated retrospectively for children undergoing ambulatory recording followed by...

Development, evaluation and implementation of video-EEG telemetry at home

PURPOSE To describe the development and implementation of video EEG telemetry (VT) in the patient's home (home video telemetry, HVT) in a single centre. METHODS HVT met the UK Medical Research Council definition of a complex intervention, and we used its guidance to evaluate the process of piloting, evaluating, developing and implementing this new clinical service. The first phase was a feasi...

Hypnosis provoked pseudoseizures: a case report and literature review.

Only a few studies have been reported in which suggestion was used to provoke pseudoseizures (PS). In these studies PS were video EEG monitored, and saline injections were administered as placebo. This method may be somewhat unethical and carries a low success rate. The authors, two child psychiatrists (GZ and DS) and a neurologist (NG), applied hypnosis to provoke PS which were monitored by vi...
