Periodic lateralized epileptiform discharges (PLEDs) in herpetic encephalitis.

چکیده
An 86-year-old male presented with confusion, fever and tonicclonic seizures. CSF analysis showed lymphocytic pleocytosis and high protein count, leading to initiation of acyclovir. EEG revealed PLEDs in the left hemisphere (figure 1C) and MRI demonstrated left hippocampal and insular hyperintensities (figure 1A, 1B). Herpes simplex virus (HSV) encephalitis was confirmed by PCR. PLEDs represent periodic sharp discharges lateralized to one hemisphere usually due to acute neuronal injury1,2. Since not all patients present seizures, the acronym LPDs (Lateralized Periodic Discharges) may be more accurate3. Besides encephalitis, several etiologies produce PLEDs, including stroke, metabolic disarrangements, brain tumors, intracranial hemorrhage, anoxic encephalopathy, among others1, 2. Nevertheless, HSV encephalitis should always be considered and CSF studies obtained in the appropriate clinical scenario4.

متن کاملPeriodic lateralized epileptiform discharges (PLEDs) as the sole electrographic correlate of a complex partial seizure.
We describe a patient in whom the only electrographic manifestation of a complex partial seizure recorded by video-EEG telemetry was periodic lateralized epileptiform discharges (PLEDs) with a left anterior temporal emphasis. The abnormality, which persisted throughout the whole recorded seizure, lends support to the claim that PLEDs can be an ictal phenomenon.
متن کاملPeriodic lateralized epileptiform discharges: association with seizures
The clinical features and EEGs of 45 consecutive patients (40 adults and 5 children) who had periodic lateralized epileptiform discharges (PLEDs) were reviewed to determine the relationship between seizures and PLEDs. Focal encephalitis and ischemic stroke were the most frequent underlying processes for adult patients. All of the children, but only six of the adults, had long-lasting cerebral d...
متن کاملSPECT in periodic lateralized epileptiform discharges (PLEDs): a case report on PLEDs.
Periodic lateralized epileptiform discharges (PLEDs), which are known as unusual electroencephalogram (EEG) patterns, are described in a patient who had stroke and seizures. This patient underwent Tc-99m HMPAO (hexamethyl propylene amine oxime) brain single photon emission computed tomography (SPECT) imaging both during PLEDs on EEG and after the cessation of PLEDs. The initial SPECT study reve...
متن کاملPeriodic lateralized epileptiform discharges as manifestation of pneumococcal meningoencephalitis
Periodic Lateralized Epileptiform Discharges (PLEDs) are usually seen in the context of destructive structural lesions of the cortex, more frequently in acute ischemic stroke and less common in tumours and meningoencephalitis, specially herpes simplex virus. Its origin and prognosis are uncertain but it is known that PLEDs are linked to epilectic seizures, including status epilepticus. We report...
