چکیده

S8 CONSULTANT SEPTEMBER 2008 (SUPPLEMENT) ABSTRACT: Dopamine receptor agonists have played an important role in antiparkinsonian therapy since the first ergoline derivative was introduced in 1974. The non-ergoline dopamine agonists, developed later to provide the benefits of the ergolines with fewer side effects, are currently used as both monotherapy and as adjunctive therapy to treat symptoms of Parkinson disease (PD), to postpone the onset of levodopa therapy, to delay the development and minimize the severity of levodopa’s complications, and to reduce the dosage of levodopa. When the effects of dopamine agonists wane and levodopa is added, patients who receive combined dopaminergic treatment still exhibit less severe motor complications than those who started antiparkinsonian therapy with levodopa. In addition to abating the core symptoms of PD (eg, akinesia and rigidity) and delaying the onset of motor complications, the dopamine agonist pramipexole has been shown to ameliorate tremor and depressive symptoms in clinical practice.

An evidence-based review of dopamine receptor agonists in the treatment of Parkinson's disease.

Apomorphine and certain ergot alkaloids (bromocriptine, lisuride and pergolide) have been available for several decades; for the last few years, they were joined by newer dopamine agonists (cabergoline, pramipexole and ropinirole) most of them are non-ergolines. Each of these dopamine agonists has its own pharmacological characteristics and occupies a place in the pharmacotherapy of Parkinsons ...

Pramipexole in Parkinson's disease. A short-term study using the combined levodopa-dopamine agonist test.

The aim of our study was to investigate the efficacy of pramipexole in advanced parkinsonian patients by means of an acute stimulation test. We studied the motor effects of pramipexole in fluctuating parkinsonian patients by comparing the response to acute levodopa with the response to levodopa + pramipexole. The adjunct of pramipexole to levodopa increased the time spent on from 136 +/- 22.3 t...

Pramipexole use and the risk of pneumonia

BACKGROUND Patients with Parkinson's disease have an elevated risk of pneumonia and randomized trials suggest that this risk may be increased with the dopamine agonist pramipexole. It is uncertain whether pramipexole or other dopamine agonists increase the risk of pneumonia. METHODS We used the United Kingdom's General Practice Research Database (GPRD) to identify users of anti-parkinsonian d...

Cardiac valve regurgitation with pergolide compared with nonergot agonists in Parkinson disease.

BACKGROUND Although most studies have suggested an increased risk of valvulopathy (primarily regurgitation) with pergolide mesylate use, one study suggested that this problem may also occur with use of the non-ergot-derived dopamine agonists pramipexole dihydrochloride and ropinirole hydrochloride. OBJECTIVE To determine if cardiac valve regurgitation occurs more commonly in patients with Par...

Adenosine A2A Receptor Antagonists Do Not Disrupt Rodent Prepulse Inhibition: An Improved Side Effect Profile in the Treatment of Parkinson's Disease

Parkinson's disease (PD) is characterized by loss of dopaminergic neurons in the substantia nigra. Current treatments for PD focus on dopaminergic therapies, including L-dopa and dopamine receptor agonists. However, these treatments induce neuropsychiatric side effects. Psychosis, characterized by delusions and hallucinations, is one of the most serious such side effects. Adenosine A(2A) recept...
