Preservation of gait biomechanics during offloading treatment of diabetic foot ulcers

چکیده

Background Gold standard for the management of diabetic foot neuropathic ulcers is the irremovable total contact cast, however evidence of equal clinical effectiveness was proved for the removable walker OPTIMA DIAB [1]. Priority of this healing treatment is the offloading of the ulcerated foot area; however, the healing process may last for months, and eventual changes in gait biomechanics may entail risks for the contralateral foot, leg joints, or even spine. Aim of this study is to setup a reliable methodology to optimize the use of such offloading devices.

Influence of Offloading Devices’ Size and Induced Limb Length Discrepancy upon Gait and Perceived Comfort of Individuals at Risk of Diabetic Foot Ulceration

INTRODUCTION Offloading devices help heal diabetic foot ulcers by taking mechanical pressure off the injured foot during weight bearing activities [1]. While offloading devices are an effective treatment measure, their designs give little consideration to the gait alterations or postural instability they induce [1, 2]. Although less frequently used than removable cast walkers (RCW), non-removab...

Footwear and offloading interventions to prevent and heal diabetic foot ulcers: Adapted IWGDF guidance

Introduction: Foot ulcer is one of the greatest complications of diabetes. If there is no effective pressure reduction in the ulcer, healing may be delayed. In this regard, the use of offloading methods for the prevention and treatment of foot ulcers in diabetic patients is known as a common therapeutic approach. Since evidence-based recommendations should be considered for the use of the above...

Background Diabetic foot ulcers are frequently related to elevated pressure under a bony prominence. Conservative treatment includes offloading with orthopaedic shoes and custom made orthotics or plaster casts. While casting in plaster is usually effective in achieving primary closure of foot ulcers, recurrence rates are high. Minimally invasive surgical offloading that includes correction of f...
