Allogeneic stem cell transplantation for adult acute lymphoblastic leukemia: when and how.

چکیده
A llogeneic hematopoietic stem cell transplantation (HSCT) with a myeloablative conditioning regi-men is considered the most potent post-remission antileukemic therapy in adult acute lymphoblastic leukemia (ALL). 1,2 However, an adequate balance should be established between its curative potential, the disadvantages (transplant-related mortality, late complications and reduced quality of life) and the improved outcome of the current chemotherapy regimens. 3-7 Large prospective trials, 8 several meta-analyses of randomized trials 9,10 and modeling studies 11 have concluded that allogeneic HSCT with myeloablative conditioning is of benefit for high-risk adult patients in first complete remission. The benefit of HSCT in patients with standard-risk features is controversial. Although the largest randomized trial in adult ALL so far showed a significant advantage of HSCT in patients with standard-risk-ALL, 8 the results of the current pedi-atric-inspired protocols are better than those from the

Background : Graft versus host disease (GVHD) is among major complications of allogeneic hematopoietic stem cells transplantation, and also is an important factor affecting the outcome of transplantation. An increased incidence of GVHD has been suggested following allogeneic peripheral blood stem cells (PBSC) transplantation, however, how this affects survival is not yet well clear. In this stu...

This retrospective study reported the outcome of 97 adult acute lymphoblastic leukemia patients who received a reduced-intensity conditioning allogeneic stem cell transplantation. With a median follow-up of 2.8 years, two year overall-survival, leukemia-free survival and non-relapse mortality were significantly better in patients transplanted in first complete remission (CR1, 52+/-9%; 42+/-10%;...

This study aimed to determine the impact of tyrosine kinase inhibitors given pre- and post-allogeneic stem cell transplantation on long-term outcome of patients allografted for Philadelphia chromosome-positive acute lymphoblastic leukemia. This retrospective analysis from the EBMT Acute Leukemia Working Party included 473 de novo Philadelphia chromosome-positive acute lymphoblastic leukemia pat...

BACKGROUND A minority of patients with adult acute lymphoblastic leukemia who relapse are rescued. The aim of this population-based study was to assess the results of reinduction treatment and allogeneic stem cell transplantation in patients in second complete remission. DESIGN AND METHODS Between 2003-2007, 76 adults (<66 years) with relapsed acute lymphoblastic leukemia (Burkitt's leukemia ...
