چکیده

D uring the last 5 years, there has been a tremendous interest in the development of new devices for percutaneous treatment of peripheral and coronary artery obstructive arterial disease.1 As during the development of balloon angioplasty, many of these devices were first tested in peripheral arteries before their use in coronary arteries. Although this stage of new device development may no longer be necessary, it did serve to increase the interest in peripheral angioplasty as well as to generate some controversy about the appropriate indications for peripheral angioplasty. The key scientific issue is to determine the appropriate roles for these new devices relative to the existing treatment modalities of conventional balloon angioplasty and bypass surgery. Ideally, randomized trials would

Overview of new technologies for lower extremity revascularization.

Lower extremity peripheral arterial occlusive disease poses a unique challenge to traditional angioplasty-based endovascular therapies. The diffuse nature of lower extremity atherosclerotic disease, the presence of chronic total occlusions, poor distal runoff, and the presence of critical limb ischemia all have contributed to the disappointing results of balloon angioplasty for complex infraing...

Reporting standards for clinical evaluation of new peripheral arterial revascularization devices.

Endoluminal revascularization devices (balloon angioplasty, atherectomy, laser recanalization, stents) have typically been evaluated in small, poorly controlled series. This has led to overly slow acceptance of some techniques (1) because of doubts by the medical community regarding long-term durability of balloon percutaneous transluminal angioplasty (PTA) versus surgery, and overly rapid acce...

Role of Atherectomy Devices in the Treatment of Lower Extremity Peripheral Arterial Disease

Introduction Atherosclerosis is a systemic disease affecting all major vascular beds. Risk factors for atherosclerosis, and therefore, for PAD, include active smoking, advance age, dyslipidemia, diabetes mellitus, and hypertension. Due to continuous increase in aging population and diabetes, incidence of peripheral arterial disease will continue to increase. While atherosclerotic coronary disea...

ANGIOPLASTY AND STENTING OF CAROTID ARTERY STENOSIS WITH EMBOLIC PROTECTION DEVICES

Background: Carotid artery stenting (CAS) has recently been recommended as an alternative to carotid endarterectomy (CEA) by some clinicians. Objective: This study was designed to evaluate the success rate and in-hospital and 30-day adverse events in our first experiences in Iran for CAS with protection devices, to document our results and guide further use of CAS. Methods: From December 2...

The Current State of Endovascular Intervention for Peripheral Arterial Disease

The rising incidence of peripheral arterial disease (PAD) has resulted in a concomitant increase in complications such as critical limb ischemia and lower-extremity amputation. Although a full spectrum of therapies exists for femoropopliteal PAD, there is not yet a well-defined consensus on the role of a specific type of endovascular modality that is best suited for treatment. Although percutan...
