چکیده

The incidence of ulcerative colitis (UC) is increasing in adults and children worldwide. Despite the advances in drug therapy, surgery is also an important treatment. Total proctocolectomy (TPC) with ileal pouch anal anastomosis (IPAA) is the gold standard surgical treatment of UC since its introduction in 1978 by Parks and Nicholls. Laparoscopic TPC with IPAA, as a new approach for UC patients, has been applied and demonstrated to be feasible and safe. Similar to many other laparoscopic surgeries, laparoscopic TPC with IPAA benefits the patients with better outcomes in terms of cosmesis, pain and early recovery, especially in young patients. As for the surgeons, laparoscopy can provide a broad field of vision, which facilitates the surgery in the deep pelvic as well as with less tissue damage. In general, the surgery involves total colectomy, rectal dissection, pouch creation and anastomosis of pouch to the anal canal.

Robotic-assisted laparoscopic surgery for restorative proctocolectomy with ileal J pouch-anal anastomosis.

Restorative proctocolectomy (RP) with ileal pouch-anal anastomosis (IPAA) is the surgical procedure of choice for chronic ulcerative colitis (CUC). Robotic-assisted laparoscopic surgery (RALS) has been shown to have its greatest merits in colorectal procedures involving the pelvis. The aim of this study was to evaluate the safety and feasibility of RP with IPAA using an innovative robotic techn...

Technique of laparoscopic-assisted total proctocolectomy and ileal pouch anal anastomosis in children and adolescents: a single center's 8-year experience.

BACKGROUND Laparoscopic techniques have been applied to restorative proctocolectomy since the early 2000s. We have employed a technique for laparoscopic-assisted total proctocolectomy (TPC) and ileal pouch anal anastomosis (IPAA) for the treatment of children with ulcerative colitis (UC). METHODS A total of 68 children underwent laparoscopic-assisted TPCs at our institution from June 2003 to ...

Ileal-pouch-anal anastomosis after restorative proctocolectomy in patients with ulcerative colitis or familial adenomatous polyposis.

BACKGROUND/AIMS Restorative proctocolectomy is the "golden standard" in surgical treatment of ulcerative colitis and familial adenomatous polyposis. The two alternative techniques of ileal-pouch-anal anastomosis include hand-made suture and double line stapled suture. The aim of the study was the analysis of postoperative complications and functional results of the two types of anastomosis. M...

Proximal diversion at the time of ileal pouch-anal anastomosis for ulcerative colitis: current practices of North American colorectal surgeons.

PURPOSE Pelvic sepsis is a serious complication after ileal pouch-anal anastomosis for ulcerative colitis that may lead to pouch failure or poor function. Although a temporary loop ileostomy may be created at the time of ileal pouch-anal anastomosis to prevent or minimize the consequences of an anastomotic leak, research has suggested that an ileostomy can be safely omitted in selected patients...
