Post-aortic left innominate vein with aortic arch aneurysm.

چکیده
Post-aortic left innominate vein (PALIV) is a rare abnormality passing over the aortic arch. A 70-year-old male was diagnosed with PALIV and distal aortic arch aneurysm (Fig. 1). Total arch replacement was successfully performed using antegrade selective cerebral perfusion of 20 8C. PALIV was preserved by complete exposure and retraction (Fig. 2). Fig. 2. Intraoperative surgeon's view: (A) post-aortic left innominate vein (PALIV) (*) located on the anterior surface of the distal aortic arch aneurysm (An) along the left vagus nerve (arrow), passing in front of the ligamentum arteriosum. (B: after graft replacement) A 26-mm quadrifurcated graft was used for graft replacement. No division of PALIV was required. Fig. 1. Three-dimensional CT: (A) post-aortic left innominate vein (PALIV) and a distal aortic arch aneurysm of 65 mm. PALIV passed in front of the aneurysm, below the aortic arch, and behind the ascending aorta before entering the superior vena cava at the junction with the right atrium.

We report a case of an uncommon thoracic aorta anomaly-right aortic arch with aberrant left innominate artery arising from Kommerell's diverticulum-that went undiagnosed until adulthood.

erally from the aortic arch. Enlargement of the nipple is usually due to dilation of the left superior intercostal vein as it heads for the left innominate vein lateral to the aortic arch. Where the superior vena cava or, occasionally, the inferior vena cava-is obstructed, the aortic nipple becomes prominent and enlarged (1-4); according to Friedman et al. (1), a nipple larger than 4.5 mm in di...
متن کاملInnominate truncal and arch blowout with left hemiparesis and right hemothorax followed by delayed cheese-wire perforation of innominate graft
We present the case of a 68 year old Caucasian woman, in extremis, with left hemiparesis and right hemothorax, in hypovolemic shock, secondary to a blow-out of a large penetrating ulcer at the junction of innominate trunk and aortic arch. She underwent interposition graft replacement of innominate trunk and repair of aortic arch, on cardiopulmonary bypass, employing total circulatory arrest and...
متن کاملNeonatal repair of right interrupted aortic arch, aberrant left subclavian artery, ventricular septal defect and retroaortic innominate vein.
We report a rare case of neonatal biventricular repair of a right interrupted aortic arch (type B), with an aberrant left subclavian artery, ventricular septal defect and retroaortic innominate vein in a 4-week-old, 2.7 kg neonate with DiGeorge syndrome. The patient had an unremarkable postoperative recovery. We discuss the anatomy of this rare congenital anomaly, its surgical implications and ...
متن کاملAgenesis of the Left Internal Carotid Artery in the Right Aortic Arch with Isolation of the Left Innominate Artery Associated with Ruptured Cerebral Aneurysm: Case Report
As we occasionally encounter congenital blood vessel abnormalities, their variety, embryology, and associated clinical problems must be understood. We report a case of a 50-year-old man with subarachnoid hemorrhage due to a ruptured cerebral aneurysm at the internal carotid-anterior choroidal artery bifurcation who manifested extremely rare congenital anomalies, i.e., a right aortic arch, isola...
