Congenital Thrombocytopenic Purpura Treated by Exchange Transfusion.

چکیده
The occurrence of thrombocytopenic purpura in babies born to mothers with idiopathic thrombocytopenia is a well recognized though uncommon clinical entity. Robson and Walker (1951) were able to collect from the literature only 19 definite and nine probable examples of this association. The suggestion of Epstein, Lozner, Cobbey and Davidson (1950) that this type of congenital thrombocytopenic purpura results from the transfer across the placenta of a maternal factor capable of causing thrombocytopenia has recently received support with the demonstration of platelet agglutinins in the serum of both mother and child (Harrington, Sprague, Minnich, Moore, Aulvin and Dubach, 1953; Vanderbroucke and Verstraete, 1955; Tullis, 1956; Schoen, King and Duane, 1956). In view of these findings it might be expected that exchange transfusion of affected infants would, by removing the acquired platelet antibodies, shorten the period of thrombocytopenia and so lessen the risk of haemorrhage and death. We report below observations on a baby with congenital thrombocytopenic purpura who was treated in this manner.

Thrombotic thrombocytopenic purpura and dose of plasma exchange.
Four patients with thrombotic thrombocytopenic purpura (TTP) were treated by plasma-exchange transfusion, three of whom recovered completely. Because previous reports in the literature describing exchange transfusion as treatment for TTP have demonstrated variable success rates, particular attention was given to "dose" and frequency of plasma exchange. Evans blue dye studies established a measu...

BACKGROUND Twice-daily plasma exchange has been used for patients with thrombotic thrombocytopenic purpura (TTP) who are unresponsive to treatment with daily plasma exchange for many years but with no clear evidence of effectiveness. STUDY DESIGN AND METHODS The 18 years' experience of The Oklahoma TTP-HUS (hemolytic-uremic syndrome) Registry, 1989 through 2006, with twice-daily plasma exchan...
متن کاملExchange transfusion for neonate with haemolytic uremic syndrome.
INTRODUCTION Haemolytic uremic syndrome (HUS) is one of the most common causes of acute renal failure in children but it is uncommon in newborns. To our knowledge only five cases have been reported so far (probably underreported). The known modalities of treatment include transfusion of plasma and plasmapheresis. We report a case of neonatal HUS for whom we performed an exchange transfusion to ...
متن کاملDeficiency of ADAMTS13 and thrombotic thrombocytopenic purpura.
Thrombotic thrombocytopenic purpura, characterized by the presence of systemic hyaline thrombi in the arterioles and capillaries, is a potentially fatal disease that responds to plasma infusion or exchange. Recent studies have demonstrated that a metalloprotease in the normal plasma cleaves endothelial von Willebrand factor to a series of multimers. A deficiency of the protease, due to autoimmu...

The rationale for immunosuppressive therapy of thrombotic thrombocytopenic purpura (TTP) was established by observations that TTP may be caused by autoantibodies to ADAMTS13. Patients with high-titer autoantibodies to ADAMTS13 may have a higher mortality, and survivors may require prolonged plasma exchange therapy in spite of adjunctive glucocorticoid treatment. More intensive immunosuppressive...
