Palmoplantar keratoderma ("tripe palms") associated with primary pulmonary adenocarcinoma.

چکیده
A 61 year old woman was investigated for a 12 month history of progressive thickening of the skin of her palms and soles. She had also complained of diarrhoea, exertional breathlessness and cough, and weight loss. There was no relevant past medical history or family history, but she had a 45 pack year history of cigarette smoking. Physical examination revealed diffuse hyperkeratosis of the palms and soles (fig 1), digital clubbing, and right sided chest signs. Her chest radiograph and thoracic CT scan (fig 2) showed a mass in the right upper lobe. This was confirmed by percutaneous biopsy to be a primary adenocarcinoma of the lung. Additional imaging excluded metastatic disease and detailed gastrointestinal investigations were all normal or negative. The keratoderma was managed initially with 10% salicylic acid in Vaseline with some response. As the tumour was inoperable on the grounds of lung function, the patient received systemic chemotherapy. This resulted in a reduction in size of the primary tumour associated with a marked improvement in the cutaneous appearance. Acquired non-familial palmoplantar keratoderma—some-times referred to as ''tripe palms''—is strongly associated with underlying neoplasia. 1 2 At least 90% of cases occur in patients with an internal malignancy, most commonly of the gastrointestinal tract or lung. Pulmonary tumours are usually non-small cell, with squamous carcinomas described most frequently. Figure 1 Diffuse keratoderma of (A) the right palm and (B) the left sole. Figure 2 Section of contrast enhanced thoracic CT scan showing a large lobulated mass in the right upper lobe extending towards the right hilum.

Receptor tyrosine kinase and p16/CDKN2 expression in a case of tripe palms associated with non-small-cell lung cancer.
BACKGROUND Tripe palms is a descriptive term for a cutaneous paraneoplastic keratoderma. Tripe palms are frequently associated with gastric and pulmonary carcinoma. The pathogenetic mechanism remains unknown. OBJECTIVE To determine the influence of receptor tyrosine kinases, which are both expressed in pulmonary carcinomas and in human skin, we performed expression studies on epidermal growth...

Palmoplantar keratoderma of Sybert (PPK of Sybert) or Sybert’s keratoderma was first documented by Virginia Sybert in 1988. Due to the high degree of similarity, it was previously considered to be Greither’s keratoderma, an established entity at that time. Currently, clinical and ultrastructural studies distinguish between the two disorders. Sybert’s keratoderma is an extremely rare type of ker...
متن کاملKeratoderma Palmoplantaris Transgrediens (Mal de Meleda): A case report
Mal de meleda (Keratoderma plamoplantaris transgrediens) is a rare autosomal recessive form of palmoplantar keratoderma with hyperkeratosis of palms and soles, which appears soon after birth and progressively involves other areas (Transgrediens) of the skin especially dorsal aspects of hands and feet. We report a 20-year-old woman with mal de meleda with some unusual clinical features, i....

Malignant acanthosis nigricans is a rare paraneoplastic skin syndrome mostly associated with gastric adenocarcinoma. Florid cutaneous papillomatosis and tripe palms syndrome are considered to be abortive clinical variants of acanthosis nigricans. Clinical manifestations include pruritic, hyperkeratotic and hyperpigmented plaques with a subsequent formation of velvety papillomas in the involved ...
