Diabetic glomerulosclerosis.

چکیده

I would like to show first the glomerulus of a 92-year-old man who did not have diabetes. It is perfectly normal. I want to emphasize that in the glomerulus there is not necessarily any ageing phenomenon per se. With the electron microscope there may be a slight increase in the thickness of the basement membrane of the glomerular capillary, but it is not visible by light microscopy. In the normal glomerulus the afferent arteriole breaks up into capillary loops which spread out to form the lobules of the glomerulus. The central portion of each lobule or group of capillary loops is called the mesangial area (fig. 1). The capillary loops themselves are lined with endothelial cells, consisting of nuclei which protrude into the lumen, and a widely spread-out thin layer of fenestrated cytoplasm lining the capillary lumen. The endothelial cell is separated from the epithelial cell by a thin layer of basement membrane, and this basement membrane connects with little processes of similar material between adjoining mesangial cells. Outside the basement membrane are the epithelial cells, each with a wide extension of foot processes, forming a network around the capillary, with a tiny canaliculi between each foot process.

Diabetic glomerulosclerosis can be the pathogenesis of refractory diabetic macular edema

BACKGROUND Kidney failure provoked by glomerulosclerosis leads to fluid filtration deficits and other disorders of kidney function. Refractory diabetic macular edema (DME) can be another warning sign of glomerulosclerosis in diabetic patients. CASE A 40-year-old Saudi male presented with macular edema that was refractory to all possible medical and surgical ophthalmic interventions in both ey...

The Morphologic Patterns of Diabetic Nephropathy in Koreans

Background : Diabetic nephropathy is the most common cause of end-stage renal disease and it has various pathologic features. We investigated the clinicopathologic differences between the histologic classes of diabetic nephropathy. Methods : A total of 46 patients with diabetic nephropathy were evaluated. Morphologically, the renal lesions were divided into three categories: class 1, diffuse or...

Mangosteen peel extract (Garcinia mangostana L.) as protective agent in glucose-induced mesangial cell as in vitro model of diabetic glomerulosclerosis

Objective(s): This study aims to evaluate the activity of mangosteen peels extract (MPE) as protection agent on induced-glucose mesangial cells (SV40 MES 13 cell line (Glomerular Mesangial Kidney, Mus Musculus)).  Materials and Methods: MPE was performed based on maceration method. Cytotoxic assay was performed based on MTS (3-(4,5-dimethylthiazol-2-yl)-5-(3-carboxymethoxyphenyl)-2-(4-sulfophen...

Prospective study on renal outcome of IgA nephropathy superimposed on diabetic glomerulosclerosis in type 2 diabetic patients.

BACKGROUND AND METHODS In order to examine the clinical outcome of IgA nephropathy (IgAN) superimposed on diabetic glomerulosclerosis in type 2 patients we studied 36 Chinese patients (26 men, 10 women), who were recruited for renal biopsy when they had proteinuria of more than 1 g/day. Twenty-seven had isolated diabetic glomerulosclerosis and nine had IgAN superimposed on diabetic glomeruloscl...

Nodular glomerulosclerosis in a non-diabetic hypertensive smoker with dyslipidemia.

Nodular glomerulosclerosis may be idiopathic or develop associated with diabetes mellitus, membranoprolipherative glomerulonephritis, light or heavy chain deposits, amyloidosis, fibrillary or immunotactoide disease, and Takayasu's arteritis. Histological features of idiopathic nodular glomerulosclerosis are similar to the Kimmelstiel-Wilson changes. Recent evidence points to the role of hypergl...

From fibrosis to sclerosis: mechanisms of glomerulosclerosis in diabetic nephropathy.

Progression of diabetic nephropathy to end-stage kidney disease is mediated by a host of processes, but none is as important as the gradual, inexorable scarring of the renal glomerulus, known as glomerulosclerosis. Hence, a host of studies over the decades have attempted to elucidate the molecular mechanisms that lead to this chronic sclerosing condition so that effective therapies and preventa...
