Gastric and duodenal ulcer and risk of bladder cancer.

چکیده
To the Editors: In the Health Professional Follow-up Study, Michaud et al. (1) found a 55% increased risk of bladder cancer for subjects with a history of gastric ulcer. Previous epidemiologic investigations on the issue are limited to patients who had undergone gastric surgery (2, 3). To provide further information on the topic, we report here data from a case-control study of bladder cancer, conducted in northern Italy between 1985 and 1992 (4). The study included 727 cases (617 males, 110 females) with histologically confirmed, incident, transitional cell bladder cancer and 1,067 controls (769 males, 298 females) admitted for acute, nonneoplastic, nonurological, or genital tract diseases. Cases and controls were questioned by trained interviewers during their hospital stay. The proportion of refusals was <5%. Odds ratios (OR) and 95% confidence intervals (CI) were obtained using multiple logistic regression, including terms for age, sex, center, and factors a priori identified as relevant in bladder carcinogenesis (5). Table 1 shows the distribution of cases and controls according to history of gastric and duodenal ulcer. The multivariate ORs were 1.19 for gastric ulcer and 1.14 for duodenal ulcer. Bladder cancer risk increased with time since first diagnosis of gastric ulcer (OR for latency z30 years, 1.50) and was highest when gastric ulcer was diagnosed between ages 30 and 44 years (OR, 1.83) and before 1955 (OR, 2.03). When we considered the latency categories 15 to 29 and z30 years together, the OR for z15 years was 1.37 (95% CI, 0.83-2.25). For duodenal ulcer, the highest ORs were 1.62 for latency between 15 and 29 years, 1.41 for ulcer diagnosis between 30 and 44 years, and 1.35 for diagnosis between 1965 and 1974. In support of an association between gastric ulcer and bladder cancer, we found a 19% increased risk among subjects with gastric ulcer, a direct association with latency, and consistent results with the Health Professional Follow-up Study. However, none of the ORs was significant, the OR was increased also for duodenal ulcer (although with no pattern for latency), and some residual confounding from smoking was possible. Nevertheless, for gastric ulcer, we found an increased risk of bladder cancer also among nonsmokers (OR, 1.23; 95% CI, 0.69-2.16). Finally, we combined our results with those from the Health Professional Follow-up Study, finding a pooled relative risk of bladder cancer of 1.47 (95% CI, 1.10-1.95) for history of gastric ulcer. Risk was highest for subjects diagnosed with gastric ulcer before 1955 (relative risk, 1.83; 95% CI, 0.91-3.67) and lowest from 1975 (relative risk, 1.06; 95% CI, 0.62-1.83), thus supporting the hypothesis of an association and of a trend in risk with latency.

Peptic ulcer disease and the risk of bladder cancer in a prospective study of male health professionals.
Helicobacter pylori is a risk factor for gastric and duodenal ulcers, but gastric ulcers generally occur in individuals who have low acid production and diffuse gastritis, whereas duodenal ulcers are more likely to occur with high acid output and antrum-predominant gastritis. Low acid production, gastritis, and ulcer healing each contribute to poor antioxidant absorption, oxidative stress, and ...
متن کاملPrevalence of EPIYA motifs in Helicobacter pylori strains isolated from patients with gastroduodenal disorders in northern Iran
Background: Cytotoxin-associated gene A (CagA)-positive strains of Helicobacter pylori are associated with gastroduodenal diseases. Evidences have suggested that the type of H pylori CagA EPIYA motifs may be associated with specific disorders (i.e., gastritis, peptic ulcer, or gastric cancer). We investigated the prevalence of different EPIYA motifs (A, B, C, or D) in H. pylori strains is...
متن کاملCancer occurrence in a cohort of patients treated with cimetidine.
The possible carcinogenic effects of antisecretory agents used in the treatment of gastric and duodenal ulcer were investigated in a population based cohort study of 16,739 patients who were prescribed the H2-antagonist cimetidine between 1977 and 1981. An excess risk for gastric cancer was observed, with a relative risk of about 10 in the first year after beginning use of the drug, which decre...

Prevalence of helicobacter pylori was evaluated by urease test in 513 patients undergoing GI endoscopy, in hospital No1 Kerman university of medical sciences and health services from June 1992 through April 1993. H. Pylori was detected in 56.9℅ of tested samples by urease test. There was no significant correlation with sex, age and cigarette smoking. Prevalence of H.pylori was 16℅ in control gr...
متن کاملLong‐term risk of gastrointestinal cancers in persons with gastric or duodenal ulcers
Peptic ulcer predicts gastric cancer. It is controversial if peptic ulcers predict other gastrointestinal cancers, potentially related to Helicobacter pylori or shared lifestyle factors. We hypothesized that gastric and duodenal ulcers may have different impact on the risk of gastrointestinal cancers. In a nationwide cohort study using Danish medical databases 1994-2013, we quantified the risk ...
