Embolectomy for stroke with emergent large vessel occlusion (ELVO): report of the Standards and Guidelines Committee of the Society of NeuroInterventional Surgery.

چکیده
In 2013, there were three major trials that were published comparing outcomes of treatment of acute ischemic stroke with IV tPA to interventional methods: IMS III, MR RESCUE, and SYNTHESIS. These trials concurrently failed to show the superiority of endovascular therapy compared to IV tPA for the treatment of acute ischemic stroke. There has been much discussion following the publication of these trials. The three main criticisms were the heavy use of intra‐arterial tPA and early‐generation devices with limited reperfusion efficacy (use of outdated technology); failure to use vascular imaging to confirm the presence of large vessel occlusion amenable to endovascular therapy, and the slow initiation of endovascular intervention.

متن کاملDecreasing procedure times with a standardized approach to ELVO cases.
BACKGROUND Embolectomy is the standard of care for emergent large vessel occlusion (ELVO), and needs to be done as quickly as possible for the best possible outcomes. Optimization of workflow and process is certainly paramount. One aspect of this is process improvement to standardize as much as possible the procedure in order to decrease variability among operators, which breeds familiarity for...
متن کاملStroke vision, aphasia, neglect (VAN) assessment—a novel emergent large vessel occlusion screening tool: pilot study and comparison with current clinical severity indices
BACKGROUND Identification of emergent large vessel occlusion (ELVO) stroke has become increasingly important with the recent publications of favorable acute stroke thrombectomy trials. Multiple screening tools exist but the length of the examination and the false positive rate range from good to adequate. A screening tool was designed and tested in the emergency department using nurse responder...
متن کاملImplications of limiting mechanical thrombectomy to patients with emergent large vessel occlusion meeting top tier evidence criteria.
BACKGROUND Recent guidelines for endovascular management of emergent large vessel occlusion (ELVO) award top tier evidence to the same selective criteria in recent trials. We aimed to understand how guideline adherence would have impacted treatment numbers and outcomes in a cohort of patients from a comprehensive stroke center. METHODS A retrospective observational study was conducted using c...
