Low-molecular-weight heparins in clinical practice.

چکیده
BACKGROUND Low-molecular-weight heparin (LMWH), because of its efficacy and ease of use, is an attractive alternative to unfractionated heparin in treatment and prophylaxis of venous thromboembolism. METHODS Using MEDLINE, I searched for relevant clinical trials using LMWH. I carefully reviewed these trials, allowing development of recommendations for use of LMWH in clinical practice. RESULTS A large amount of data is available for evaluating the use of LMWH in treatment of venous thromboembolism and in prophylaxis against venous thromboembolism. Less data are available concerning the utility of these agents in cardiovascular and cerebrovascular disorders. CONCLUSIONS Based on available literature, LMWH should be strongly considered as therapy for acute deep venous thrombosis, though Food and Drug Administration (FDA) approval for this indication is currently pending. These agents are also excellent pharmacologic prophylactic agents in orthopedic surgery patients. Recent data suggest use in unstable angina in place of heparin. Other indications for use are not as well defined at this time.

Pharmacological and clinical differences between low-molecular-weight heparins: implications for prescribing practice and therapeutic interchange.
Thanks to their predictable pharmacokinetics and ease of use, low-molecular-weight heparins (LMWHs) have established uses in the prevention and treatment of thrombotic diseases and as a replacement for unfractionated heparin (UFH). Although LMWHs as a class have similar antithrombotic effects, they comprise a diverse group of agents with distinct biochemical and pharmacological profiles. In lig...
متن کاملLow-molecular-weight heparins in the treatment of venous thromboembolism
Recent studies have indicated that certain low-molecular-weight heparins given subcutaneously may replace continuous intravenous unfractionated heparin for the treatment of venous thromboembolism. Low-molecular-weight heparins have a predictably high absorption rate when given subcutaneously and they do not require laboratory monitoring. These characteristics of low-molecular-weight heparin the...
متن کاملAnticoagulants in coronary artery disease.
Anticoagulant therapy for acute coronary syndromes is becoming more complex as newer agents are added to unfractionated heparin and warfarin. The anticoagulants used in current clinical practice are low molecular weight heparins, direct thrombin inhibitors, and heparinoids. Properties of and recent clinical trial data regarding these newer anticoagulants are reviewed in reference to current Ame...

متن کاملPrevention of Venous Thromboembolism in Major Orthopedic Surgery: Bayesian Network Meta-Analysis of 21 Randomized Trials Evaluating Unfractionated Heparins, Low-Molecular Weight Heparins, and New Oral Anticoagulants
Background: In major orthopedic surgery, prevention of venous thromboembolism has been based on Unfractionated Heparins (UFHs) over the past decades, then on LowMolecular Weight Heparins (LMWHs), and on New Oral Anticoagulants (NOACs) more recently. To summarize the comparative effectiveness of UFHs, LMWHs, and NOACs in this clinical indication, we applied Bayesian network meta-analysis to the ...
متن کاملOversulfated chondroitin sulfate: impact of a heparin impurity, associated with adverse clinical events, on low-molecular-weight heparin preparation.
Heparin, a widely used anticoagulant, is being rapidly displaced by low-molecular-weight heparins. Recently, certain lots of heparin have been associated with anaphylactoid-type reactions resulting from contamination with oversulfated chondroitin sulfate. This impurity has also contaminated low-molecular-weight heparins obtained by chemical and enzymatic depolymerization of heparin. The sensiti...
