چکیده

The evaluation of hypertrophic cardiomyopathy (HCM) involves the careful consideration of diseases that can mimic its appearance, such as aortic stenosis, poorly controlled hypertension, and infiltrative processes such as cardiac amyloidosis. Fabry disease, an inherited deficiency of the enzyme α galactosidase A, in particular, has recently received much attention for new and nearly curative treatments. 2 Exogenous administration of the deficient enzyme has been shown to clear microvascular endothelial deposits, which are integral to disease manifestation, in the kidney, skin, and heart. Interestingly, isolated cardiac Fabry disease may exist without other overt manifestations. In one study, 3% of male subjects with myocardial hypertrophy were found to harbour this enzyme deficiency, whereas another study showed that approximately 4% of patients referred to a tertiary centre for evaluation of HCM actually had evidence of Fabry disease. If this storage disease were misdiagnosed as HCM, patients who could be treated effectively with medication might be subjected to aggressive procedures such as septal ablation or surgical septal myectomy. We sought to determine the frequency of Fabry disease among patients who underwent surgical myectomy for the relief of symptomatic obstructive HCM.

Familial hypertrophic obstructive cardiomyopathy with the GLA E66Q mutation and zebra body

BACKGROUND Fabry disease is caused by mutations in the α-galactosidase A (GLA) gene, which is located in X-chromosome coding for the lysosomal enzyme of GLA. Among many gene mutations, E66Q mutation is under discussion for its pathogenicity because there is no clinical report showing pathological evidence of Fabry disease with E66Q mutation. CASE PRESENTATION A 65-year-old Japanese female was...

The subaortic tendon as a mimic of hypertrophic cardiomyopathy

Originally described by Brock and Teare, today hypertrophic cardiomyopathy is clinically defined as left (or right) ventricular hypertrophy without a known cardiac or systemic cause, such as systemic hypertension, Fabry's disease or aortic stenosis. Also appreciated today is the enormous genotypic and phenotypic heterogeneity of this disease with more than 300 mutations over more than 24 genes, ...

Apical Hypertrophic Cardiomyopathy in a Case with Chest Pain and Family History of Sudden Cardiac Death: A Case Report

Hypertrophic cardiomyopathy (HCM) is the most common genetic cardiovascular disease, which is caused by a multitude of mutations in genes encoding proteins of the cardiac sarcomere (1). Apical hypertrophic cardiomyopathy (AHCM) is an uncommon type of HCM. The sudden cardiac death is less likely to occur in the patients inflicted with AHCM (2). Herein, we presented the case of a 29-year-old man ...

A 71-year-old female who was diagnosed with nonobstructive hypertrophic cardiomyopathy since 1999 presented with dyspnea and severe edema on both legs. For the management of her symptom, cardiac surgery including tricuspid annuloplasty, Maze operation and right atrial reduction plasty was performed. During follow-up after cardiac surgery, a plasma α-galactosidase activity was checked for the sc...
