The role of Rheumatoid Factor and Anti-Cyclic Citrullinated Peptide Antibody in Diagnosis of Rheumatoid Arthritis

چکیده
Background: Rheumatoid Factor (RF) occurs in 7090% of patients with established rheumatoid arthritis (RA). Anti-cyclic citrullinated peptide antibody (Anti-CCP) detection may be used to confirm the diagnosis of RA in patients with uncharacterized chronic inflammatory arthritis. This study is an attempt to evaluate the role of RF and anti-CCP in the diagnosis and prognosis of rheumatoid arthritis. Methods: Blood samples were obtained from 55 patients with established RA and 55 healthy controls. Anti-CCP and RF were measured by ELISA and nephelometry. The patient's demographics, disease duration, DMARDs usage, ESR and radiographic abnormalities were recorded. Results: Fifty five RA patients and 55 healthy age and sex matched controls were enrolled. Odd ratio (OR) was 87.42 (CI=11.2-680.1) for RF and 27.48 (6/08-124/09) for AntiCCP. Five patients had used more than 3 DMARDs; all of them were RF positive and 4 were also anti-CCP positive. Eleven patients had erosions in their radiographs; 10 of them were seropositive for RF and 9 positive for anti-CCP. Conclusion: Our study suggests that despite lack of specificity, RF continues to be a central part of the definition of RA due to favorable sensitivity profile and the combined use of RF. Also, anti-CCP is a more powerful diagnostic and prognostic tool.

Abstract Background: Rheumatoid Arthritis, (RA) is a chronic inflammatory autoimmune disease with a progressive course requiring early diagnosis and treatment. The value of anti-cyclic citrullinated peptide antibodies (Anti-CCP) in the diagnosis of RA has been shown in several studies. The present study was designed to determine and compare the diagnostic value of Anti-CCP and rheumatoid fac...

Anti-cyclic peptide antibodies (CCP/ACPA) are directed towards synthetic citrullinated peptides and are specific markers in diagnosis of Rheumatoid arthritis. This study aimed to compare the values and acuracy of the test in anticyclic citrullinated peptides (Anti-CCP /ACPA) antibodies, rheumatoid factor (RF), C-reactive protein (CRP) and disease activity index (PASI) in early diagnosis of untr...

Background: Controlling the pulmonary involvement, as one of the most common extra-articular consequences in rheumatoid arthritis (RA), can improve the life quality and survival rate of patients. This study aimed to evaluate the correlation between disease-related inflammatory factors and the severity and type of lung involvement in patients, which is a practical step toward early detection and...

This study aimed to evaluate the role of Anti-Cyclic Citrullinated Peptide (anti-CCP) antibody in comparison to Cartilage oligomeric matrix protein (COMP) in Rheumatoid Arthritis (RA) patients as predictors of the disease activity and cartilage destruction. The study included 60 patients &10 apparently healthy subjects. They were divided into 4 groups. Group 1: consisted of 20 patients with est...

متن کاملAnti-cyclic citrullinated peptide antibody is a good indicator for the diagnosis of rheumatoid arthritis
OBJECTIVES Anti-cyclic citrullinated peptide (CCP) antibody has recently been used in the classification of rheumatoid arthritis (RA). This antibody is more specific than rheumatoid factor (RF) for the diagnosis of RA. The study objectives were to determine the sensitivity, specificity, positive and negative predictive values of anti-CCP in RA diagnosis. METHODOLOGY Eighty RA patients and 80 ...
