چکیده

Study objectives: To investigate the etiology of posthyperventilation (post-HV) hypoxemia following voluntary hyperventilation (VHV), we examined the effects of hypocapnic (hypo-C02) and isocapnic (iso-C02) VHV on the hypoxic ventilatory response (02-response) and on the sensation of breathlessness during the 02-response. Methods: 02-responses and visual analog scale (VAS) scores for estimating breathlessness in 10 normal subjects during the 02-response under iso-C02 conditions and under hypo-C02 condi¬ tions immediately following voluntary maximal HV of 3 min duration were examined. Results: Although there was no significant difference in the post-HV ventilation levels following hypo-C02 vs iso-C02 VHV, the VAS scores at the start of the 02-response following hypo-C02 VHV (30.2±24.2 mm) were significantly higher (p<0.05) than the VAS scores at the start of the 02-response following iso-C02 VHV (13.7±8.4 mm). However, VHV did not have a significant effect on the 02-response at 2 min after theVHV when the arterial 02 saturation (Sa02) was below 90%. The nonsteady-state hypo-C02 induced by VHV greatly attenuated the 02-response below 90% Sa02 and VAS scores at 70% Sa02. Conclusions: Elevated VAS scores immediately following the hypo-C02 VHV, which might be independent of actual breathing levels, and the attenuation of the 02-response following the hypo-C02 VHV were not due to input from lung and chest wall mechanoreceptors induced by the hyperpnea itself, but rather to the hypo-C02 induced by hyperpnea. Effect of episodic hypoxia on the susceptibility to hypocapnic central apnea during NREM sleep.

We hypothesized that episodic hypoxia (EH) leads to alterations in chemoreflex characteristics that might promote the development of central apnea in sleeping humans. We used nasal noninvasive positive pressure mechanical ventilation to induce hypocapnic central apnea in 11 healthy participants during stable nonrapid eye movement sleep before and after an exposure to EH, which consisted of fift...

Changes in respiratory control during and after 48 h of isocapnic and poikilocapnic hypoxia in humans.

Ventilatory acclimatization to hypoxia is associated with an increase in ventilation under conditions of acute hyperoxia (VEhyperoxia) and an increase in acute hypoxic ventilatory response (AHVR). This study compares 48-h exposures to isocapnic hypoxia (protocol I) with 48-h exposures to poikilocapnic hypoxia (protocol P) in 10 subjects to assess the importance of hypocapnic alkalosis in genera...

Ventilatory, cerebrovascular, and cardiovascular interactions in acute hypoxia: regulation by carbon dioxide.

This study examined the effect of high, normal, and uncontrolled end-tidal Pco(2) (Pet(CO(2))) on the ventilatory, peak cerebral blood flow velocity (V(p)), and mean arterial blood pressure (MAP) responses to acute hypoxia. Nine healthy subjects undertook, in random order, three hypoxic protocols (end-tidal Po(2) was held at eight steps between 300 and 45 Torr) in conditions of hypercapnia, iso...

Sustained hyperoxia stabilizes breathing in healthy individuals during NREM sleep.

The present study was designed to determine whether hyperoxia would lower the hypocapnic apneic threshold (AT) during non-rapid eye movement (NREM) sleep. Nasal noninvasive mechanical ventilation was used to induce hypocapnia and subsequent central apnea in healthy subjects during stable NREM sleep. Mechanical ventilation trials were conducted under normoxic (room air) and hyperoxic conditions ...
