چکیده

Fig. 1. Barium swallow depicting an epiphrenic diverticulum. Preoperative manometry revealed significant tertiary contractions with intact primary peristalsis. The lower esophageal sphincter relaxed normally which excluded the diagnosis of achalasia. Fig. 2. Resected diverticular sac demonstrating retained potassium supplementation pills. The patient had undergone a successful diverticulectomy, distal esophageal myotomy, and Belsey partial fundoplication through a left thoracotomy. There was minimal mucosal damage or peridiverticular inflammation as these were enteric coated pills. Following surgery, the patient’s hypokalemia resolved.

Giant esophageal epiphrenic diverticulum: presentation and treatment

Incomplete esophageal myotomy and early recurrence of an epiphrenic diverticulum.

The established management of epiphrenic diverticula combines diverticulectomy with myotomy of the distal esophagus including the lower esophageal sphincter. We report a patient with prompt recurrence of an epiphrenic diverticulum after laparoscopic transhiatal diverticulectomy associated with esophagomyotomy and partial posterior fundoplication due to an incomplete myotomy.

Carcinoma arising from an epiphrenic diverticulum: a frequently misdiagnosed disease.

Epiphrenic diverticulum is rare, comprising of about 10% of all esophageal diverticula. Carcinoma arising within such a diverticulum is even less common. We report on two patients with squamous cell carcinoma arising from an epiphrenic diverticulum. Some features of malignant change related to the epiphrenic diverticula were misdiagnosed or missed in our cases and other reports due to lack of c...

Laparoscopic Transhiatal Treatment of Large Epiphrenic Esophageal Diverticulum

We report the case of a 73-year-old woman with a large symptomatic esophageal epiphrenic diverticulum, diffuse nonspecific esophageal dysmotility, and a hiatal hernia. METHODS Surgery was indicated by the patient's symptoms, the size of the diverticulum (maximum diameter 10 cm), and the associated...

Laparoscopic approach in the treatment of large epiphrenic esophageal diverticulum

Epiphrenic diverticulum of the lower third of the esophagus is a relatively rare disorder. We present the case of a large, 7.5 cm diameter esophageal epiphrenic diverticulum treated by the laparoscopic approach. Surgery was indicated by the severity of the patient's symptoms and size of the diverticulum. A laparoscopic transhiatal diverticulectomy with a myotomy and Dor fundoplication was carri...
